腹腔镜手术治疗对良性妇科肿瘤患者机体免疫功能的研究

Study on Immune Function of Patients with Benign Gynecologic Tumor by Laparoscopic Surgery

目的研究腹腔镜手术治疗对良性妇科肿瘤患者机体免疫功能的影响。方法方便选取该院妇产科2014年6月—2016年3月收治的72例良性妇科肿瘤患者为研究对象,将其按照单双号分为观察组和对照组,各36例。其中对照组患者采用开腹手术治疗,观察组患者采用腹腔镜手术治疗。对两组患者手术前后机体免疫功能状况进行观察、分析。结果两组患者术前身体各项指标差异无统计学意义(P>0.05)。术后观察组患者TNF-α水平为(43.71±28.29)ng/L,术前TNF-α水平为(29.11±16.78)ng/L,治疗前后比较差异有统计学意义(P<0.05);术后对照组患者在CD8+、CD4+、CD4+/CD8+、CD3+等方面与观察组患者比较差异有统计学意义(P<0.05);对照组患者术后TNF-α水平为(55.13±31.71)ng/L,两组比较差异有统计学意义(P<0.05);术前两组患者IL-4、IL-2水平差异无统计学意义(P>0.05)。术后3d对照组患者IL-4、IL-2水平与术前比较差异有统计学意义(P<0.05);与观察组患者比较差异有统计学意义(P<0.05)。结论腹腔镜手术治疗良性妇科肿瘤患者方面的效果比较显著,不会对患者机体免疫功能造成严重的影响,可以作为临床治疗该类疾病的首选方法,值得在能开展腹腔镜手术医院推广。

Objective This paper tries to study the effect of laparoscopic surgery on immune function in patients with benign gynecologic tumor. Methods 72 cases of gynecologic cancer patients in this hospital from June 2014 to March 2016 were convenient selected and divided into the observation group and the control groups, with 36 cases in each group. Patients in the control group were treated with open surgery, and the observation group received laparoscopic surgery. The immune function of the two groups before and after the operation were observed and analyzed. Results There was no difference between the two groups of patients before the operation and no statistically significant difference（P0.05）. TNF alpha level for patients after operation in the observation group was（43.71±28.29） ng/L, preoperative level of TNF alpha was（29.11±16.78） ng/L, and there was statistically significant difference before and after treatment（P0.05）; There were statistically significant differences between the groups in CD8＋, CD4＋, CD4＋/CD8＋, CD3＋ etc（P0.05）. The TNF-αlevel in the control group was（55.13±31.71）ng/L, and the differences in the two groups were statistically significant（P0.05）. The levels of IL-4, IL-2 were not statistically significant（P0.05） in the two groups before operation. The difference between IL-4, IL-2 was statistically significant in 3 days after operation. The comparison with the observation group was statistically significant（P0.05）. Conclusion Laparoscopic surgery for the treatment of benign gynecological cancer patients has significant effect, does not cause serious effects on patients with organic immune function, can be used as the preferred method for clinical treatment of this disease, and it is worth promotion.
