食管癌术后吻合口瘘的X线及CT诊断分析

Analysis of X-ray and CT Diagnosis of Anastomotic Leakage after Operation of Esophageal Carcinoma

目的:探讨和分析食管癌术后吻合口瘘的X线及CT诊断和治疗方法。方法:将我院在2015年2月至2017年2月期间接受治疗和实施食管癌根治术的80例患者选取为本次的研究对象。其中有5例患者出现了术后吻合口瘘,所有的吻合口瘘患者均采用X线及CT诊断方法。结果:患者口服对比剂后,再实施X线造影和16排CT扫描,可以明确的诊断出瘘的大小、位置以及分布情况,同时也根据CT的特征性征象对因各种原因引起的隐匿性瘘患者做出初步的判断,为隐匿性吻合口瘘。结论:食管癌术后会出现的严重并发症就是吻合口瘘,由于引起每个阶段的吻合口瘘的原因都相对较为复杂,常规的X线一般很难持续摄影,但CT具有相对的优势,可以明确的检查出瘘口的具体位置、大小、吻合口周围的情况以及胸腔内的积液。此外,常规的X线造影以及口服对比剂之后再进行CT扫描有非常重要的意义。可以对患者的状况作出初步的诊断,有利于患者的临床治疗,因此,对于患者的生命质量存在很大意义。

Objective： To explore and analyze the X-ray and CT diagnosis and treatment of postoperative anastomotic fistula of esophageal carcinoma. Methods： 80 patients who received treatment and implementation of radical esophageal cancer from February 2015 to February 2017 were selected as the subjects of this study. There were 5 patients with postoperative anastomotic fistula, and all patients with anastomotic fistula were treated with X-ray and CT. Results： After oral administration of contrast agent, X-ray and 16-slice CT scans were performed to confirm the size, location and distribution of the fistula, and the seizure of the fistula due to various causes fistula patients make a preliminary judgment for occult anastomotic fistula. Conclusion： The serious complication of postoperative esophageal cancer is anastomotic fistula. The reason of anastomotic fistula caused by each stage is relatively complicated. The conventional X-ray is difficult to continue to be photographed, but CT has the opposite advantages, you can clearly check the specific location of the fistula, size,anastomosis around the situation and the pleural effusion. In addition, conventional X-ray imaging and oral contrast agents after CT scan has a very important significance. Can make a preliminary diagnosis of the patient＇s condition, is conducive to the clinical treatment of patients, therefore, for the quality of life of patients there is great significance.
