摘要 目的探讨高频超声检测软组织内异物的临床价值。方法选择2016年1—6月江苏大学附属上海第八医院普外科收治的215例疑有软组织异物残留的入院患者,术前均行高频超声检测,以实时超声引导下手术探查结果为金标准,分析高频超声检测软组织内异物的临床价值。结果 215例患者手术共探查307处软组织,实际有异物数275个,实际无异物数32个。术前高频超声检测262个异物阳性,误诊3个,45个异物阴性,漏诊16个,其灵敏度为94.2%,特异度为90.6%,漏诊率为5.8%,误诊率为9.4%,正确率为93.8%,阳性似然比为10.046,阴性似然比为0.064,约登指数为0.848。结论高频超声廉价、安全、高效,其检测软组织内异物的正确率高,具有重要的临床价值,可指导手术实施。 Objective To investigate the clinical value of high frequency ultrasound in detecting foreign bodies in soft tissue. Methods From January 2016 to June 2016, 215 hospitalized patients suspected of " soft tissue foreign body residue" and treated in Department of General Surgery, Shanghai Eighth Hospital Affiliated to Jiangsu University were selected. All patients underwent preoperative high frequency ultrasound examination. Ultrasound guided surgical exploration resuhs as the gold standard, the clinical value of high frequency ultrasound in detecting different kinds of foreign bodies in soft tissue was analyzed. Results In 215 cases, 307 soft tissues were examined. There were 275 foreign bodies confirmed in the operation, and there were no foreign bodies confirmed in the 32 operation. Two hundred sixty - two positive foreign bodies were detected by high frequency ultrasound before operation, 3 were misdiagnosed, 45 foreign bodies were negative, and 16 were missed. The sensitivity was 94.2% , specificity was 90. 6% , the missed diagnosis rate was 5.8% , the misdiagnosis rate was 9.4% , the accuracy was 93.8% , the positive likelihood ratio was 10. 046, negative likelihood ratio was 0. 064, Youden index was 0. 848. Conclusion High frequency ultrasound is cheap, safe and efficient. It has high clinical value in the detection of foreign bodies in soft tissue.
