瓜蒌薤白半夏汤联合增强型体外反搏对不稳定性心绞痛患者心脏室壁运动的影响

Effect of Gualou Xiebai Banxia Decoction combined with enhanced external counterpulsation on ventricular wall motion in patients with unstable angina

目的观察瓜蒌薤白半夏汤联合增强型体外反搏(EECP)对不稳定性心绞痛(UA)患者心脏室壁运动的影响。方法选择2014年9月至2016年9月在本院内科住院的90例UA患者为研究对象,随机分为观察组和对照组各45例。观察组患者接受西药治疗+瓜蒌薤白半夏汤+EECP治疗,对照组患者接受西药治疗+EECP治疗。比较两组患者观察治疗前后加拿大心血管病学会(CCS)心绞痛分级、心脏室壁运动情况。结果治疗后,两组患者的CCS心绞痛分级均显著降低,且观察组患者的CCS心绞痛分级评分(1.69±0.56)分,显著低于对照组患者(2.00±0.54)分,差异有统计学意义(t=-2.183,P=0.033)。两组患者心脏室壁运动评分均较治疗前降低,且观察组心脏室壁运动评分(1.00±0.60)分显著低于对照组患者(1.37±0.58)分,差异具有统计学意义(t=-2.428,P=0.018)。两组患者在EECP治疗过程中,收缩压110~150 mm Hg,舒张压60~90 mm Hg,心率60~95次/min,D/S比值1.2~1.3。结论瓜蒌薤白半夏汤联合体外反搏治疗能够改善不稳定性心绞痛患者心脏室壁运动,减轻心绞痛症状。

Objective To observe the effect of Gualou Xiebai Banxia decoction combined with enhanced external couterpulsation （EECP） on ventricular wall motion in patients with unstable angina （UA）.Methods 90 patients （hospitalized in our hospital during September 2014 and September 2016） with UA were randomly divided into two groups ： the experimental group （ n = 45 ） and the control group （ n = 45 ）. The patients in the experimental group were treated with western medicine plus Gualou Xiebai Banxia decoction and EECP. And the others in the control group were treated with western medicine and EECP. Canadian Cardiovascular Society （CCS） grading of angina pectoris and ventricular wall motion were observed before and after treatment.Results The CCS grading of angina pectoris was lower in the experimental group than that in the control group after treatment （ 1.69 ＋0.56 vs 2.00＋0.64）, t=-2. 183,P=0.033. The ventricular wall motion was improved in both two groups after treatment. And the score of ventricular wall motion in the experimental group was significantly lower than that in the control group （1.00＋0.60 vs 1.27＋0.58）, t=-2.428, P=0.018. Conclusions Gualou Xiebai Banxia decoction combined with external counterpulsation therapy can improve the ventricular wall motion and improve the symptoms of angina for patients with unstable angina.
