谷氨酰胺联合大黄治疗重度烧伤的临床效果观察 被引量:1

Curative Effect of Glutamine Combined with Dahuang ( Radix et Rhizoma Rhei) in Treatment of Severe Burns

摘要 目的:观察并探讨谷氨酰胺联合大黄治疗重度烧伤的临床效果及其安全性.方法:将2013年12月~2016年12月本院收治的重度烧伤患者62例随机分为观察组和对照组,每组各31例.观察组患者采用烧伤常规治疗+谷氨酰胺+大黄治疗,对照组患者采用烧伤常规治疗.比较2组患者治疗前后血浆白蛋白(ALB)、前白蛋白(P-ALB)、转铁蛋白(TF)、内毒素、白介素-17(IL-17)、IL-10及干扰素-γ(IFN-γ)水平以及创面愈合时间、并发症情况.结果:治疗7天后观察组患者血浆ALB、P-ALB、TF显著高于对照组,差别具有统计学意义(P〈0.05),治疗后观察组患者内毒素、IL-17及IFN-γ水平低于对照组,IL-10水平高于对照组,差别具有统计学意义(P〈0.05).观察组患者创面愈合时间均优于对照组(P〈0.05).结论:谷氨酰胺联合大黄治疗能够显著改善重度烧伤患者的营养状况,加快创面愈合,抑制炎症反应发生. Objective: To assess the curative effect and securityof glutamine combined with Dahuang (Radix et Rhizoma Rhei ) in treatment of severe.bums. Methods: Sixty -two severe bums patients who were treated from December 2013 to December 2016 at Baogang hospital were randomly divided into either an observation group (n = 31) or a control group (n = 31). The control group was given onventional treatment, and the observation group was given glutamine combined with Dahuang (Radix et Rhizoma Rhei). Curative effects, setum albumin(ALB), prealbumin (P- ALB),tranderrin(TF) ,endotoxin , interleukin 17 (IL- 17) ,IL- 10 and interferon-γ ( IFN -γ)were compared for the two groups. Results:: On the seventh day after treatment, ALB、P - ALB,TF were significantly higher in the observation group compared with the control group (P 〈 0. 05 ), endotoxin, IL- 17, IFN -γ were significantly lower in the observa- tion group compared with the control group ( P 〈 0.05 ). IL - 10 was higher in the observation group than in the control group ( P 〈 0. 05 ). On the twenty - one day after treatment, Wound healing of observation group were better than control group. ( P 〈 0. 05 ). Conclusion: Glutamlne combined with Dabuang (Radix et Rhizoma Rhei) is able to significantly improve nutritional status , healing and inhibiting inflammatory response in patients with severe burns.
