闭合复位经第四掌骨平行克氏针内固定治疗第五掌骨颈骨折

Closed reduction and internal fixation with parallel Kirschner wires through the fourth metacarpal bone for treatment of the fifth metacarpal neck fractures

目的探讨闭合复位后经第四掌骨平行克氏针内固定治疗第五掌骨颈骨折的手术方法和临床效果。方法自2014年5月至2016年12月，对15例第五掌骨颈骨折患者采用经第四掌骨闭合复位克氏针内固定治疗，术后6周拔除内固定行功能训练。结果13例骨折取得解剖复位或接近解剖复位，2例轻度成角畸形，术后随访2-3个月，按中华医学会手外科学会上肢部分功能评定试用标准评定：优13例，良2例。结论闭合复位经第四掌骨平行克氏针内固定治疗第五掌骨颈骨折，操作简便，固定可靠，费用低，疗效满意。

Objective To investigate the surgical technique and clinical effects of closed reduction and internat fixation with parallel Kirschner wires through the fourth metacarpal bone in the treatment of the fifth metacarpal neck fractures. Methods From May 2014 to December 2016, 15 cases of the fifth metacarpal neck fractures were treated by closed reduction and internal fixation with parallel Kirschner wires through the fourth metacarpal bone. Internal fixators were removed 6 weeks after the operation followed by early postoperative functional training. Results Anatomical reduction or near-anatomical reduction was achieved in 13 cases. Mild angular deformity occurred in 2 cases. All the patients were follow-up for 2 to 3 months. According to the functional assessment criteria of the upper limb formulated by the Hand Surgery Society of the Chinese Medical Association, the results were excellent in 13 cases, good in 2 cases. Conclusion Closed reduction and internal fixation with parallel Kirschner wires through the fourth metacarpal bone for treatment of the fifth metacarpal neck fractures can achieve satisfactory clinical effects due to convenient operation, reliable fixation and low cost.
