摘要
目的:探讨血清脂联素测定在不稳定型心绞痛和稳定型心绞痛中的临床研究。方法:选取确诊为心绞痛的患者120例,根据病症类别分为UAP组(65例)和SAP组(55例),并选取健康人50例作为正常对照组。记录对比3组的临床资料,以及脂联素水平与心血管事件发生率的关系。结果:UAP组和SAP组脂联素均比对照组明显降低,UAP组降低更多(P<0.05);UAP组和SAP组的低密度脂蛋白、总胆固醇、高敏C反应蛋白和肌钙蛋白指标均比对照组明显提高,UAP组提高更多(P<0.05);UAP组、SAP组和对照组的体重指数、舒张压和收缩压指标对比差异无统计学意义(P>0.05)。低脂联素组发生心血管事件的概率大于高脂联素组(χ2=4.82,P<0.05)。结论:UAP组和SAP组患者的血清脂联素指标均低于健康人群,UAP组更低;脂联素水平与多项临床指标和心血管事件发生率相关,可作为UAP和SAP临床诊断的重要测定指标。
Objective: To study the clinical determination of serum adiponectin( APN) for unstable angina pectoris( UAP) and stable angina pectoris( SAP). Methods: 120 cases of angina pectoris( AP) patients as the research objects were divided into two groups,65 of which were unstable angina pectoris( UAP) and 55 of which were stable angina pectoris( SAP). And 55 cases of healthy people who were not diagnosed as angina pectoris( AP) were chosen as control group. The clinical data in three groups were recorded and compared,as well as the relationship between the level of serum adiponectin( APN) and incidence rate of cardiovascular disease,CVD. Results: The adiponectin( APN) in unstable angina pectoris( UAP) and stable angina pectoris( SAP) groups was apparently decreased than that of the control group,especially in UAP group( P〈0. 05).. The low-density lipoprotein,( LDL),total cholesterol,serum hypersensitive C reactive protein( hs-CRP) and cardiac troponin I( cTnI) in UAP and SAP groups were clearly increased than those of the control group,especially in UAP group( P〈0. 05).. There was no statistically significant difference in the body mass index( BMI) and systolic pressure and diastolic pressure in three groups( P〉0. 05). The probability of cardiovascular disease,CVD in lower serum adiponectin( APN) was more than that in the higher serum adiponectin( APN)( X^2= 4. 82,P〈0. 05). Conclusions: The index of serum adiponectin in patients with SAP group and UAP group are lower than those of patients without coronary heart disease,while much lower in UAP group; And adiponectin level is associated with multiple clinical indexes and incidence of cardiovascular events,which can be used as important measurement index of UAP and SAP clinical diagnosis.
