摘要
目的比较优化脉冲技术与超脉冲点阵CO_2激光治疗增生性瘢痕的临床疗效与安全性。方法回顾性分析2014年1月至2016年1月,皮肤增生性瘢痕患者137例的临床资料,将其分为两组,对照组应用优化脉冲技术进行治疗;治疗组应用超脉冲点阵CO_2激光治疗。随访半年,比较两组患者的治疗效果和不良反应发生率。结果两组患者瘢痕治疗后充血、色素沉着改善,瘢痕逐渐变平滑。治疗组患者的总有效率(93.2%),明显高于对照组(79.7%),两组比较差异具有统计学意义(P<0.05)。两组患者的半年内的复发率比较,差异无统计学意义(P>0.05)。治疗组14例(占19.2%)出现色素沉着等不良反应,对照组未出现明显不良反应。结论应用超脉冲点阵CO_2激光治疗皮肤增生性瘢痕的疗效优于优化脉冲技术,且治疗次数较少。而优化脉冲技术治疗患者的耐受度高、不良反应较少、恢复较快,对瘢痕的充血及色素不均的改善较明显。
Objective To observe the effect of optimal pulsed technology and ultra-pulsed CO2 fractional laser in treating hypertrophic scars and investigate more safe and effective therapy for hypertrophic scars.Methods Totally 137 patients with hypertrophic scars were randomly divided into two groups: the control group and treatment group.The patients in the control group were given the treatment of optimal pulsed technology and those in the treatment group were given the ultra-pulsed CO2 fractional laser treatment. By the time of half a year's follow-up,the effect and side effect of two groups were compared and evaluated by comparing pre-and-post treatment photographs.Results The appearance of the scars was improved after the treatment: hyper pigment and erythema gradually vanished and surface be-came smooth etc. The total effective rate of the treatment group was significantly higher than that of the control group( P〈0. 05). The difference between two groups in the recurrence rate within half a year was not statistically significant( P〉0. 05). Side effects like hype-pigment occurred to 14 patients in the treatment group. No obvious side effect was found within the control group.Conclusions For treating hypertrophic scars,ultra-pulsed CO2 fractional laser is superior to optimal pulsed technology in therapeutic effect,but the incidence rate,patients' tolerance and recovery speed of the latter is more desirable.
