摘要
目的 分析我院2011?2013年间大肠埃希菌临床分离及对抗菌药物的耐药性变化趋势,为临床合理使用抗生素提供实验依据.方法 收集我院2011?2013年分离的大肠埃希氏菌,按照《全国临床检验操作规程》进行鉴定;采用 CLSI推荐的(K-B)琼脂扩散法进行药敏实验和结果评价.结果 897株大肠埃希氏菌对临床常用抗菌药物均产生不同程度耐药性,3年平均耐药率大于50%的有氨苄西林(91. 16%),哌啦西林(78. 76%) ,头孢唑啉(73. 29%) ,头孢呋辛(71. 1 3 % ),头孢曲松(69. 4 3 % ),头孢噻肟(69. 28%),四环素(66. 16%),环丙沙星(61. 90%)、左氧氟沙星(59. 97%),复方新诺明(64. 01%).耐药率低的分别是亚胺培南(1. 22%),哌拉西林/他唑巴坦(3. 93%),阿米卡星(7. 09%),头孢西丁(13. 06%).3年间我院B-内酰胺类药物耐药率呈逐年下降趋势,哌啦西林/他唑巴坦、亚胺培南等耐药率较平稳.结论 大肠埃希菌是引各类临床感染的重要病原菌之-,其耐药性不容忽视,加强细菌耐药性连续监测,指导临床合理使用抗生素,控制细菌耐药性的产生和蔓延.
Objective To investigate the clinical distribution and trends of antimicrobial resistance of Escherichia coli isolates from 2011to2013 so as to provide the basis for seasonable use of antibiotics. Methods The Escherichia coli from The second hospi-tal of yichang from 2011to2013were collected and identified by referring to national Guided to Clinical Laboratory procedures, The drug susceptibility testing was performed by K-B methods recommended by CLCI. The testing results were assessed. Results To-tally 897 strains of Escherichia coli have become resistant to commonly use antimicrobial agents to various degrees. Among these av-erage drug resistance rate exceeded 50% included ampicillin( 91. 16 % ) piperacillin( 78. 76 %) , cefazolin( 73. 29 %) , cefaroxime( 71. 13 %) ceftriaxome(69. 43 %) , cefotaxime(69. 28%) , tetracycline(66 , 16%), ciprofloxacin(61. 90% levofloxacin( 59. 97%) sulfame- thoxazole-trimethoprim(64. 01%) . Low rate of resistance were imipenemd. 22%) , piperacillin/tazobactam(3. 93 % ) , amikacin(7. 09%) ,cefoxitin(13. 06%). During the three years,the drug resistance rate of B-Lactams antimicrobial appeared a downward trend. The drug resistance rates of imipenem and piperacillin/tazobactam remained stable. Conclusion Escherichia coli is one of the impor-tant pathogen leading various clinical infection, drug resistance can not be ignored, strengthen the continuous monitoring of bacteri-al resistance to direct the rational use of antibiotic in clinic and to control the bacterial drug resistance and spread.
