老年慢性病住院患者下呼吸道鲍曼不动杆菌的药敏情况

Drug Sensitivity of Acinetobacter Baumannii in Hospitalized Elderly Patients with Chronic Lower Respiratory Disease

目的:了解某院老年慢性病住院患者下呼吸道鲍曼不动杆菌感染的临床分布及耐药情况,及时做好防控措施。方法:收集某院2015年3月~2016年3月期间,年龄为60周岁以上有慢性病史的住院患者的痰液作为标本,从中分离出151株鲍曼不动杆菌。应用珠海迪尔细菌鉴定系统随机体外诊断试剂板进行菌种鉴定和药敏试验。结果:鲍曼不动杆菌感染主要分布在ICU、呼吸内科、神经内科。151株鲍曼不动杆菌中,除多粘菌素、米诺环素和头孢哌酮/舒巴坦以外,其他药物的耐药率均在40%以上,美洛培南的耐药率为72%,亚胺培南耐药率为61.7%,阿米卡星、环丙沙星、哌拉西林/舒巴坦耐药率高达80%以上。结论:老年住院患者易感染鲍曼不动杆菌,特别是有慢性病史的重症患者,鲍曼不动杆菌对临床常用抗菌药物耐药严重,临床医生应参考药敏结果合理用药。

Objective：To learn the clinical distribution and drug sensitivity about hospitalized chronic elderly patients with lower respiratory tract infection＇Acinetobacter baumannii in our hospital and provide the evidence for the prevention and control measures without delay.Methods：Separated 151 Acinetobacter baumannii from the hospitalized patients＇ sputum samples in a hospital aged ≥60 years who had the history of chronic disease from March 2015 to March 2016.And then apply Zhuhai Deere bacterial identification system to have the strain identification and susceptibility testing in vitro diagnostic reagents board.Results：The acinetobacter baumannii infection is mainly serious in ICU,neurology,respiratory medicine.In the 151 Acinetobacter baumannii,drug resistance was more than 40% except for polymyxin,minocycline and cefoperazone/sulbactam.Acinetobacter baumanniis＇.Drug resistance of Meropenem was 72% ,imipenem resistance rate was 61.7% ,resistance rates of amikacin,ciprofloxacin,piperacillin/sulbactam were above 80% .Conclusion：The hospitalized elderly patients are more infected to the Acinetobacter baumannii easily,especially those patients with severe chronic history.The acinetobacter baumannii are very strong resistant to the clinical commonly used antibiotics.The clinicians should prescript reasonablely by referring to the rational drug sensitivity results.
