周围型空洞性肺腺癌多层螺旋CT比较研究 被引量:6

摘要 目的:探讨伴随有空洞形成的周围型肺腺癌患者多层螺旋CT(MSCT)影像特征的差异。方法:回顾分析2012年1月~2016年4月经病理证实的24例伴空洞形成的周围型肺腺癌患者的临床资料和MSCT表现。根据空洞壁的厚度不同(<4mm或≥4mm),分成薄壁空洞组(10例)与厚壁空洞组(14例),对2组的临床资料、MSCT征象进行比较分析。结果:在MSCT上超过半数的影像学征象有:病变形态不规则22例(91.6%)、胸膜凹陷20例(83.3%)、分叶征18例(75%)、内壁不光整13例(54.2%)。薄壁空洞组中,空洞内间隔的显示、血管穿行征及病灶周边磨玻璃影显示均显著高于厚壁空洞组(均P<0.05),厚壁空洞组肺腺癌患者更易出现胸膜牵拉及分叶征(P<0.05)。结论:伴薄壁空洞形成肺腺癌与厚壁空洞形成肺腺癌在影像学特征中存在一定差异。 Objective:To evaluate multi-slice CT(MSCT)features of peripheral ities.Methods :Twenty four cases of pathologically confirmed lung adenocarcinc lung adenocarcinomas with cav- containing cavities were ret- rospectively analyzed with regard to clinical data and MSCT features between Jan.2012 and April 2016.Ac- cording to the thickness of the cavity walls (〈4mm or 4mm),the lesions were divided into thin-walled or thick-walled cavity groups.Fisher exact test and ANOVA test were used for the statistical analysis.Reulls: There were 24 cases (10 with thin walled cavities, 14 with thick-walled cavities).These features accounted for more than 50% of all MSCT signs in 24 cases,including irregular shapes in 22 cases (91.6%),lobulations in 18 cases(75%),irregular inner walls in 13 cases(54.2%).The frequency of septum in airspace,the blood ves- sel passing through the airspace and ground-glass opacity around the cavity in thin-walled cavities were high- er than thick-walled cavities (P〈0.05).The frequency of lobulations and pleural in lesions with thick-walled cavities were higher than those in thin-walled cavity adenocarcinomas (P〈O.OS).Condusion:Radiological char- acteristics of the thin-walled cavities were different from the thick-walled cavity adenocarcinomas.
