γ-谷氨酰转移酶与缺血性脑卒中患者颅内外动脉粥样硬化性狭窄的相关性研究 被引量:9

摘要 目的探讨血清γ-谷氨酰转移酶(GGT)水平与缺血性脑卒中患者颅内外动脉粥样硬化性狭窄的相关性。方法选择合肥市第一人民医院神经内科住院的缺血性脑卒中患者145例,行全脑数字减影血管造影检查,根据颅内外动脉狭窄情况分成2组,其中颅内外动脉狭窄率≥50%(至少1支血管狭窄率≥50%)者为狭窄组,101例;狭窄率<50%者为对照组,44例。收集患者临床资料及血液检测情况,并分析患者血清GGT水平与颅内外动脉粥样硬化性狭窄的关系。结果狭窄组年龄、糖尿病、吸烟、丙氨酸转氨酶、GGT、TG、LDL-C和高敏C反应蛋白水平显著高于对照组,差异有统计学意义(P<0.05,P<0.01)。logistic回归分析显示,血清GGT水平是颅内外动脉粥样硬化性狭窄的独立危险因素之一(OR=1.048,95%CI:1.013~1.084,P=0.007)。随着血清GGT水平的升高,颅内外动脉粥样硬化性狭窄发生率逐渐增高(χ~2=13.170,P=0.004)。结论血清GGT水平与颅内外动脉粥样硬化性狭窄密切相关,是其发生和发展重要的危险因素。 Objective To study the association of serum GGT level with intracranial and extracra- nial atherosclerotic stenosis in ischemic stroke patients. Methods One hundred and forty-five is- chemic stroke patients who underwent DSA in our hospital were divided into stenosis group (n= 101) with their intracranial and extracranial atherosclerotic stenosis rate ≥50% and control group (n= 44) with their intracranial and extracranial atherosclerotic stenosis rate 〈50 %. Their clinical data and laboratory testing data were recorded. The association of serum GGT level with intracra- nial and extracranial atherosclerotic stenosis was analyzed. Results The age was significantly older, the incidence of diabetes mellitus was significantly higher, the history of smoking was signifi- cantly longer,the serum levels of GGT,TG, LDL-C and hs-CRP were significantly higher in ste- nosis group than in control group (P〈0.05, P〈0.01). Logistic regression analysis showed that serum GGT level was one of the independent risk factors for intracranial and extracranial athero- sclerotic stenosis in ischemic stroke patients (OR=1. 048,95 % CI..1. 013--1. 084, P=0. 007). The incidence of intracranial and extracranial atherosclerotic stenosis increased with the elevating ser- um GGT level in ischemic stroke patients (X2=13. 170,P=0. 004). Conclusion The serum GGT level is closely associated with intracranial and extracranial atherosclerotic stenosis and is thus an important risk factor for the development and progression of intracranial and extracranial athero- sclerotic stenosis in ischemic stroke patients.
