41例原发性腹膜后恶性肿瘤的诊断和治疗

Diagnosis and treatment of 41 cases of malignant primary retroperitoneal tumor.

目的 总结原发性腹膜后恶性肿瘤的诊治经验。方法 回顾性分析 4 1例原发性腹膜后恶性肿瘤病人的临床表现、影像学检查特点及手术处理。结果 初始临床表现多为腹部肿物 ,辅助检查CT诊断率为 94 7% ,肿瘤完全切除率为 4 8 8% ,完全切除组术后复发 11例 ,再手术 10例 ,6例完全切除肿瘤。结论 发现腹部无症状性肿物要考虑本病 ,CT是最重要的诊断性辅助检查 ,手术全切除及整块切除是治疗本病的重要手段 ,对复发性肿瘤应争取再手术治疗。

Objective To summarize the diagnosis and treatment experience in malignant primary retroperitoneal tumor (MPRT). Methods Forty one patients with MPRT were admitted and treated.Clinical manifestations,imaging findings and surgical managements were analyzed.Results Abdominal mass was the main symptom and sign. The diagnostic rate by CT was 94 7%.The whole resection rate was 48 8%,11 cases with whole resection had recurrence and 6 cases had whole resection again. Conclusion Abdominal mass in patients without other symptoms should be considered MPRT.CT scan is the most important method for diagnosis of MPRT.The total resection and en bloc resections are the best way of treatment. The recurrence tumor should be diagnosed early and be treated with reoperation.
