摘要
目的探讨结直肠癌患者围手术期早期肠内营养的效果。方法将110例结直肠癌手术患者随机分为对照组和观察组,各55例;对照组给予肠外营养支持疗法,而观察组则给予早期肠内营养支持疗法;比较两组患者的术后恢复情况、免疫指标及营养指标。结果术后7天,观察组的首次排气时间、住院时间及切口感染率均明显小于对照组(P<0.05),生存质量评分高于对照组(P<0.01);观察组的CRP增加水平低于对照组(P<0.01),而观察组的IgA、IgG、CD4^+、CD4^+/CD8^+增加水平则高于对照组(P<0.05);观察组的血清白蛋白、前白蛋白、转铁蛋白均高于对照组(P<0.05)。结论结直肠癌患者早期实施经口肠内营养支持疗法,可提高患者的免疫能力,改善营养状态,加快病情恢复,值得在临床推广应用。
Objective To explore the effect of early enteral nutrition support by mouth for colorectal cancer patients in the perioperative period. Methods One hundred and ten colorectal cancer patients were randomly divided into control group and observation group, respectively 55 cases. The control group was given parenteral nutrition support method, and the observation group was given enteral nutrition support method. The indexes of postoperative recovery, immunity and nutrition were compared between the two groups after operation. Results Seven days after operation, the indexes of first exhaust time, length of stay, life quality and infection rate of incisional wound in the observation group were much lower than those in the control group (P〈0. 05). The increasing level of CRP in the observation group was much lower than that in the control group (P〈0.01), while the increasing levels of Ig A, Ig G, CD4+ and CD4+/CD8+ in the observation group were much higher than those in the control group (P〈0.05). The levels of serum albumin, pre.zlhumin and transferrin in the observation group were much higher than those in the eontrol group (P〈0.05). Conclusion The early carrying out of enteral nutrition support by mouth for colorectal cancer patients could raise immune ability, improve nutritional status, and speed up disease recovery. So it deserves promotion in clinical practice.
