功能性消化不良不同亚型患者的生存质量及影响因素

Quality of life and influencing factors of functional dyspepsia in different subtypes

目的探讨功能性消化不良（FD）不同亚型患者的生存质量及影响因素。 方法对210例FD患者按照罗马Ⅲ分型标准分为三组：单纯餐后不适综合征（PDS）69例、单纯上腹痛综合征（EPS）74例和重叠型（PDS＋EPS）67例。并采用简明健康状况调查表（SF-36）、汉密顿抑郁量表（HAMD）、汉密顿焦虑量表（HAMA）、艾森克个性问卷（EPQ）、生活事件量表（LES）对他们进行调查研究。 结果①SF-36各维度的组间比较：生理机能（PF）和生理职能（RP）差异不明显；躯体疼痛（BP）、一般健康状况（GH）、精力（VT）、社会功能（SF）、情感职能（RE）、精神健康（MH）以及生理复合分（PCS）和心理复合分（MCS）中，PDS＋EPS低于PDS和EPS；在VT和MH中，PDS比EPS高，其余维度两单纯型组间差异未见统计学意义（P〉0.05）。②相关回归分析提示PCS和MCS中三型都与躯体焦虑有关联；PCS中PDS与睡眠障碍有关联，MCS中PDS与睡眠障碍和L量表，EPS与睡眠障碍和E量表，PDS＋EPS与N量表也存在关联。 结论不同亚型的FD患者的生存质量及影响因素有差异，必须针对不同亚型的患者采取不同的心理干预治疗方案，来达到治疗疾病并提高患者的生存质量的目的。

ObjectiveTo investigate the quality of life and influencing factors of functional dyspepsia（FD） in different subtypes. MethodsTwo hundred and ten patients with FD were divided into postprandial distress syndrome（PDS） , epigastric pain syndrome （EPS） and overlapping（PDS＋ EPS）. SF-36 quality of life questionnaire（SF-36）, Hamilton depression scale（HAMD）, Hamilton anxiety scale（HAMA）, the Eysenck’s Personality Questionnaire（EPQ） and the life event scale （LES） were used in the three groups. Results①There was no significant difference in physiological function（PF）or role physical （RP）among the three groups; the body pain（BP）, general health（GH）, vitality（VT）, social function（SF）, role Emotional（RE）, mental health（MH）, physical component summary（PCS）and mental component summary（MCS） in patients with PDS＋ EPS were lower than those of patients with PDS or EPS. The VT and MH, PDS were higher than those of EPS. There was no significant difference among the rest of the dimension between PDS and EPS. ②PCS and MCS in three subtypes, the influencing factors were Somatic anxiety factor. In the PCS, the influencing factor was PDS with sleep disturbance factor. In the MCS, the influencing factors were PDS with Sleep disturbance factor and L, EPS with sleep disturbance factor and E, PDS ＋ EPS with N. ConclusionsThere are significant differences in the quality of life among the three subtypes of FD patients. Different psychological intervention and treatment should be taken according to different subtypes of patients, in order to cure disease and improve their quality of lives.
