护理慢病管理门诊在心内科患者中的应用效果

Application effects of nurse-led chronic disease management clinic in patients with cardiovascular disease

目的 总结护理慢病管理门诊对心内科患者进行慢病管理的方法和效果.方法 采用方便抽样法,选取2016年1—6月在四川大学华西医院心血管慢病管理门诊就诊的300例患者,采用自制问卷调查患者的就诊满意度,同时统计2016年上半年和2015年同时段护理门诊量及复诊量,从复诊率和就诊满意度两方面对门诊效果进行评价.结果 2016年上半年复诊率为51.9%,2015年同时段复诊率为33.8%,两个时段复诊率比较差异有统计学意义（χ^2=5.58,P〈0.05）,患者对门诊管理方式的总体评价较高,满意度为95.7%.满意度较低的两个方面为疾病监测管理和情绪管理.结论 患者对慢病管理门诊管理方式总的评价较高,心血管慢病管理门诊有效地加强了对慢病患者的管理.

Objective To investigate the method and effects of nurse-led chronic disease management clinic in patients with cardiovascular disease.Methods A total of 300 patients admitted to the nurse-led chronic disease management clinic for cardiovascular disease of West China Hospital, Sichuan University from January to June 2016 were selected by convenience sampling and surveyed using self-designed questionnaire to investigate the satisfaction of patients. The nurse-led clinic volume and revisiting volume were collected in the first half year of 2015 and 2016 respectively. The effects of nurse-led clinic were evaluated with both revisiting and satisfaction rate.Results The revisiting rate in the first half year of 2016 was 51.9%, while in the first half year of 2015 was 33.8%, and there was a significant difference between the two periods（χ^2=5.58,P〉0.05）. Patients had a higher satisfaction rate of 95.7% for the chronic disease management in clinic. The two aspects with lower satisfaction rate were disease surveillance and emotion management.Conclusions Patients are satisfied with the chronic disease management in nurse-led clinic. Management of cardiovascular chronic disease clinic can effectively strengthen the management of patients with chronic disease.
