超声乳化白内障吸除人工晶状体植入术治疗闭角型青光眼的疗效分析

Curative Effect of Phacoemulsification and Intraocular Lens Implantation Used in Treating Angle-Closure Glaucoma

目的:探讨超声乳化白内障吸除人工晶状体植入术对治疗闭角型青光眼的临床疗效。方法:选取我院眼科2015年3月~2017年1月期间收治的84例(113眼)闭角型青光眼患者,随机分为观察组(超声乳化白内障吸除人工晶状体植入术治疗组)59只眼和对照组(青光眼滤过性手术)54只眼,比较两组患者术后的视力、眼压及并发症情况。结果:术后两组对比,观察组视力≥0.5的26例,0.5~0.1的27例,而视力<0.1的6例,对照组视力≥0.5的12例,0.5~0.1的31例,而视力<0.1的11例;术后观察组眼压(12.0±2.9)mm Hg,对照组眼压(14.6±3.1)mm Hg;观察组前房深度(3.2±0.9)mm,对照组(2.4±0.7)mm,对比差异具有统计学意义(P<0.05)。观察组患者的术后并发症发生率1.8%,对照组9.3%。结论:在闭角型青光眼的临床治疗当中,超声乳化白内障吸除人工晶状体植入术相对滤过手术具有视力改善好、眼压控制低、并发症少的优点。

to analyze the curative effect of phacoemulsification and intraocular lens implantation used in treating angle -closure glaucoma. Method： 84 angle -closure glaucoma cases （including 113 eyes） were selected from the hospital database, which were treated from March 2015 to January 2017. These cases were randomly arranged into the study group including 59 eyes （treated by phacoemulsification and in- traocular lens implantation） and the control group including 54 eyes （treated by glaucoma filtration sur- gery）. The post - operative vision, intraoeular pressure, and complication were analyzed. Result ： as shown by the comparison between the two groups, in the study group, there are 26 cases whose vision is no less than 0.5,27 cases＇ vision is between O. 5 and 0.1 ,and 6 cases＇ vision is lower than 0.1. In the control group, 12 cases＇ vision is no less than 0.5,31 cases＇ vision is between 0.5 and 0.1,and 11 cases＇ vi- sion is lower than 0.1. As to the intraocular pressure, the study group presents （12.0±2.9） mmHg, while the control group presents （ 14.6 ± 3.1 ） mmHg. The depth of anterior chamber showed by the study group is （3.2 ± 0.9 ） mm, while the control group is （ 2.4±0.7 ） mm. Such differences have statistic meaning （P 〈0.05 ）. The post- operative complication rate showed by the study group is 1.8% ,while the control group is 9.3 %. Conclusion ： when treating the angle - closure glaucoma, compared with the filtration surgery, phacoemulsification and intraocular lens implantation present better vision improvement, lower intraocular pressure control and less complication.
