浆细胞性龟头炎误诊为顽固性硬下疳1例

Plasma Cell Balanitis Misdiagnosed as Persisting Hard Chancre:A Case Report

患者男,53岁。龟头持续性红斑3月余,无明显自觉症状。皮损组织病理示:表皮水肿,真皮浅层毛细血管扩张,淋巴细胞和浆细胞浸润。皮肤镜检查见弥漫分布的橙黄色无结构区和迂曲的血管。RPR(-),TPPA(+)。驱梅治疗无效。综合考虑后将诊断修正为浆细胞性龟头炎。

A 53-year old man presented with persistent erythema on the glans penis and there are no obviously subjec- tive symptoms for more than 3 months. Histopathological examination demonstrated epidermal edema, the proliferation of capillaries and lymphoeytes and plasma ceils infiltration in the upper dermis. Dermatoscope showed diffuse orange-yellowish structureless areas and curved vessels. Rapid plasma reagin test was negative and Treponema pallidum particle assay was positive. The antisyphilitie treatment was not effetive. We correc- ted the diagnosis of persisting hard chancre to plasma cell balanitis.
