血红蛋白水平波动对血液透析患者住院率及病死率的影响

Impact of hemoglobin level fluctuation on hospitalization rate and mortality of hemodialysis cases

目的了解血红蛋白(Hb)水平波动对血液透析(Hd)患者住院率及病死率的影响。方法根据Hb水平,将163例Hd患者分为持续目标值内组、持续目标值外无波动组、目标值以下/以上小波动组、目标值以下/以上大波动组。比较各组生化指标、肾性贫血、住院和死亡情况,并用回归分析筛选住院或死亡危险因素。结果 6组患者Hb水平、促红细胞生成素(EPO)使用剂量及调整次数、住院情况差异有统计学意义(P<0.01或0.05)。回归分析显示Hb水平、EPO使用剂量和调整次数与住院相关(P<0.05)。结论 Hd患者住院与EPO剂量和调整次数、Hb水平波动有关。

Objective To investigate the effect of hemoglobin （Hb） level fluctuation on hospitalization rate and mortality of hemodialysis （Hd） patients. Methods Based on Hb levels, 163 Hd patients were divided into continuous intra-target value, continuous extra-target value without fluctuation, under/above-target value with small fluctuation, and under/above-target value with great fluctuation groups. The biochemical indices, renal anaemia, hospitalization and death were compared, and risk factors for hospitalization and death were analyzed by regression analysis. Results There were significant differences in lib concentration, erythropoietin （EPO） dosage and adjustment, and hospitalization among six groups （P〈0.01 or 0.05）. The regression analysis showed that Hb level and EPO dosage and adjustment were correlated with hospitalization （P〈0.05）. Conclusion The hospitalization is related to EPO dosage and adjustment and Hb level fluctuation in Hd patients.
