摘要 目的探讨胎盘生长因子(PlGF)在结核性胸腔积液中的表达及其与治疗效果的关系。方法检测56例结核性胸膜炎合并胸腔积液患者血清和胸腔积液中PlGF的表达水平,根据胸部CT检查结果将患者分为胸膜增厚组17例和未增厚组39例,分析胸腔积液中PlGF含量与胸膜增厚、胸水吸收时间及患者一般资料的关系。结果胸膜增厚组胸腔积液中PlGF水平为(1114.59±120.69)ng/L,明显高于胸膜未增厚组的(878.61±116.57)ng/L,差异有统计学意义(P<0.01),根据积液吸收时间将患者分为3组:胸水吸收时间<2周18例,2~4周30例,>4周8例,PlGF水平分别为(945.56±92.46)、(1080±152.43)、(1138±218.38)ng/L,3组比较差异有统计学意义(P<0.01),胸腔积液中PlGF表达水平与患者胸水总蛋白量呈正相关(r=0.269,P<0.05)。结论在结核性胸腔积液患者的PlGF表达水平与胸膜增厚、胸水吸收时间及胸水总蛋白量关系密切,机制可能是PlGF通过刺激胸膜血管生成和渗透性增加而促进结核性胸膜炎病情的进展。 Objective To explore the expression of placental growth factor (P1GF) in tuberculous-pleural effusion and its relation to the clinical effects. Methods The P1GF levels in the serum and pleural effusion of fifty-six cases with tuberculous (TB) pleurisy complicated with pleural effusion were detected. The patients were divided into two groups based on the degree of pachynsis pleurae detected by chest high-resolution computed tomography, 17 cases in Thickening Group and in 39cases Non-Thickening Group. The relationship of PIGF content in pleural effusion with pleural thickening, pleural effusion absorption time and general data of the patients was analyzed. Results The concentration of P1GF in effusion of Thickening Group was (1114.59± 120.69) ng/L, significantly higher than that of Non-Thickening Group ((878.61 ± 116.57) ng/L), and there was statistical difference (P〈0.01). The patients were divided into three groups based on pleural effusion absorption time: 18 cases with a pleural effusion absorption time 〈 2 weeks, 30 cases with a pleural effusion absorption time of 2-4 weeks and 8 cases with a pleural effusion absorption time 〉4 weeks. Their P1GF level was (945.56± 92.46), (1080±152.43) and (1138±218.38) ng/L, respectively, and there was statistical difference among three groups (P〈0.01); The P1GF expression level in pleural effusion was positively correlated to total protein in pleural effusion (r=0.269, P〈0.05). Conclusion The expression level of P1GF is closely related to pleural thickening, pleura/effusion absorption time and total protein in pleural effusion. The possible mechanism is that P1GF may play an important role in tuberculous-pleural development by promoting angiogenesis and permeability.
