2型糖尿病肾病与心血管钙化的相关性研究 被引量:1

摘要 目的探讨2型糖尿病肾病(T_2DN)患者血清胎球蛋白A(Fetuin A)水平与血管钙化及炎症因子的相关性。方法回顾性分析166例2型糖尿病肾病患者的病历资料,同时选择同期在我院体检的40例健康体检者作为对照;采集所有研究对象外周静脉血和尿液,观察Fetuin A、IL-1β、IL-6、FPG、Alb、TC、LDL-C、TG、Ca^(2+)、P、CRP的表达。并用B超观察颈总动脉钙化斑。结果 T_2DN各亚组与A组相比,Fetuin A蛋白表达均明显下降(P<0.05);各亚组间比较,差异具有显著性(P<0.05)。T_2DN各亚组与A组相比,钙化明显升高(P<0.05),其中E组明显高于B组(P<0.05)。VC组Fetuin A表达明显高于VNC组。Fetuin A与ALB、CHL、TG呈正相关,与LDL-C、Ca^(2+)、P、CRP、IL-1β、IL-6呈负相关。结论 T_2DN患者血清Fetuin A水平与炎症因子及血管钙化具有一定的相关性,可作为糖尿病肾病的诊断指标之一。 Objective To investigate the relationship between serum fetuin A level and vascular calcification(VC),inflammatory factors in type 2 diabetic nephropathy patients.Methods The clinical data of 166 patients with diabetic nephropathy were analyzed retrospectively,and 40 cases of physical examination at the same time in our hospital were compared.Peripheral blood samples and urine from all subjects were collected,and expressions of Fetuin,A,IL-1,beta,IL-6,FPG,Alb,TC,LDL-C,TG,Ca2+,P and CRP were observed.The carotid artery was also detected to look for the plaue with Ultrasound Doppler.Results Compared with A group,the expression of Fetuin A protein decreased significantly in T_2DN subgroups(P0.05).A mong the subgroups,the difference was significant.Compared with the A group,the calcification of the T_2DN subgroups increased significantly(P〈0.05),and the E group was significantly higher than that of the B group(P〈0.05).The expression of Fetuin A in group VC was significantly higher than that in group VNC.Fetuin A were positively related to ALB,CHL,TG,and negatively correlated with LDL-c,Ca2+,P,CRP,IL-1,beta,and IL-6.Conclusion Serum Fetuin and A levels in patients with T_2DN have some correlation with inflammatory factors and vascular calcification,which can be used as a diagnostic index of diabetic nephropathy.
