D二聚体检测联合256层CTA在缺血性结肠炎早期诊断中的价值

Value of D-Dimer Measurement Combined with 256-Slice CTA in the Early Diagnosis of Ischemic Colitis

目的探讨D二聚体检测联合256层CTA在缺血性结肠炎(IC)早期诊断中的价值。方法回顾性分析2011年6月~2016年6月在佛山市第一人民医院诊治的100例IC患者临床资料,根据血浆D二聚体水平、腹部CTA双期扫描结果、结合结肠镜的确诊时间进行对比分析。结果 D二聚体联合CTA检测阳性率分别与单纯D二聚体检测及单纯CTA阳性率相比较高,差异有统计学意义(P<0.05)。D二聚体联合CTA检测的诊断时间短于单纯CTA检测及单纯D二聚体检测,差异有统计学意义(P<0.05)。结论 D二聚体检测联合256层CTA在缺血性结肠炎早期诊断中可提高诊断效率和准确性,把诊断时间缩短至入院48h左右,为后续的结肠镜检查及病理活检提供良好的指引,为改善患者结肠缺血争取到宝贵的时间,建议作为疑诊缺血性结肠炎患者行结肠镜之前的常规诊断组合项目。

Objective To investigate the value of D-dimer measurement combined with 256-slice CTA in the early diagnosis of ischemic colitis.Methods One hundred patients with ischemic colitis who were treated in First People’s Hospital between June 2011 and June 2016 were retrospectively analyzed.Plasma D-dimer levels,CTA scan result and coloscope diagnosis time were compared and analyzed.Results The detection positive rate of D-dimer combined CTA was significantly higher than D-dimer alone and CTA alone（P〈0.05）.The detection time of D-dimer combined CTA was significantly shorter than D-dimer alone and CTA alone（P〈0.05）.Conclusion Ddimer measurement combined with 256-slice CTA can increase the efficacy and accuracy of early diagnosis of ischemic colitis, shorten the diagnosis time within 48 hours after admission, provide evidence for the subsuquent coloscope and pathology examinations, gain valuable time for improving ischemic colitis, and can be used as a conventional diagnostic means prior to coloscope examination in patients with suspected ischemic colitis.
