3D打印模型辅助后路螺钉内固定术治疗可复性寰枢椎脱位的临床应用 被引量:2

摘要 目的研究3D打印模型辅助后路内固定治疗寰枢关节脱位的方法和效果。方法采用3D打印技术建立寰枢椎3D打印模型,在模型上进行模拟置钉,获得个体化置钉数据,辅助术中置钉。结果 11例采用3D打印技术建立寰枢椎3D打印模型,根据术前CT三维重建判断,及在模型上进行模拟置钉,9例行寰椎侧块螺钉固定,7例行枢椎椎弓根螺钉固定,2例行枢椎峡部螺钉固定,2例行寰枢椎经关节螺钉固定。患者术中、术后未出现神经血管损伤等严重并发症,C1螺钉位置:0级螺钉39枚,1级螺钉1枚,C2螺钉位置:0级螺钉38枚,1级螺钉2枚。术后随访时间6~28个月,平均15.2个月。术后JOA评分显著提高,差异有统计学意义(P<0.0001)。术后复查MRI及CT,寰枢椎脱位完全复位8例,不完全复位3例。术后延髓颈髓角(CMA)明显增大,延髓腹侧受压明显减轻或消失,差异有统计学意义(P<0.0001)。结论利用3D打印模型辅助寰枢椎后路螺钉置入技术能有效提高临床手术置钉的准确性和安全性;后路螺钉间撑开复位治疗寰枢椎脱位疗效满意。
