全髋关节置换术治疗髋部骨折术后继发创伤性髋关节骨性融合的疗效分析

Total hip arthroplasty in treatment of postoperative hip traumatic hip fusion

目的探讨全髋关节置换术治疗髋部骨折术后继发创伤性髋关节骨性融合的治疗效果。方法收集2016年1月至2017年3月人院的30例髋部骨折术后继发创伤性髋关节骨性融合的患者，比较患者治疗前后Harris评分情况、治疗前前屈、后伸幅度与治疗后前屈、后伸幅度，统计总体治疗优良率、临床恢复情况以及手术后并发症发生情况。结果经过t检验，患者治疗前Harris评分为（35．12±7．12）分、治疗后Harris评分为（85．41±6．64）分，比较治疗前后Harris评分，治疗后Harris评分显著升高，差异有统计学意义（P〈0．01）；患者治疗前前屈、后伸幅度为（25．12±5．12）°、（1．52士0．25）°，治疗后前屈、后伸幅度为（77．34±8．78）°、（7．34±1．28）°，治疗前后比较，差异均有统计学意义（均P〈0．01）；治疗后髋关节功能恢复情况优良率为86．70％，恢复尚可3例，恢复较差1例；发生感染2例，静脉栓塞1例，并发症的发生率为9．68％。结论采用全髋关节置换术治疗髋关节骨性融合效果满意，有效提高了患者Harris评分，显著改善了患者髋关节前屈、后伸幅度，值得临床推广，具有借鉴性。

Objective To explore the therapeutic effect of total hip arthroplasty in the treatment of postoperative hip traumatic hip fusion. Methods 30 patients with secondary traumatic hip arthroplasty treated at our hospital from January, 2016 to March, 2017 were enrolled in the study. The scores of Harris before and after the treatment, the flexion and extension, the overall effective rate, the clinical recovery, and the postoperative complications were compared. Results The result of t test showed that the Harris score was （35.12 ±7.12） before and （85.41 ± 6.64） after the treatment, with a statistical difference （P 〈 0.01）. The flexion and extensionpatients were （25.12 ±5.12） ° and （1.52 ± 0.25）°before and （77.34v8.78）°and （7.34±1.28）° after the treatment, with statistical differences （all P 〈 0.01）. After the treatment, 86.70% patients recovered well, 3 so so, and 1 not well and 2 got infection and 1 embolism. The incidence of complications was 9.68%. Conclusion Total hip arthroplasty in the treatment of hip fusion is satisfactory and can effectively improve the patient＇s Harris score and hip flexion and extension, so it is worth bieng clinical generalized and of reference.
