改良式上颌窦内提升术的临床观察

The clinical observation of useing improved maxillary sinus augmentation

目的:探讨改良式上颌窦内提升技术在种植手术中的应用效果。方法:28例患者,共36颗种植体,可用骨高度在4~6mm,采用Summers骨凿经牙槽嵴顶提升术,术中将明胶海绵填入种植窝,待明胶海绵吸满血液,用骨凿轻柔旋转压迫明胶海绵,使海绵内血液受压流向上颌窦底黏膜下,从而分离、松解上颌窦底黏膜,提升高度为3~4mm,无植骨材料植入,同期植入种植体。术后6个月完成修复,观察修复后1年种植体周围骨稳定性,上颌窦底种植体周围骨量变化;数据用PASW statistic 18.0软件进行统计学分析。结果:36枚植体均完成修复,并负重行使功能1年,种植体周围骨稳定性良好。结论:改良式单纯上颌窦内提升术式是可行的骨增量技术。

Objective： To evaluate the effect of improved maxillary sinus augmentation technique in implant surgery. Methods： 28 patients included a total of 36 implants were seclected, with available bone height of 4 ~6mm in the maxillary sinus floor. Summers osteotome was use in the surgery. During the operation, gelatin sponge was filled into planting nest and sucked blood. Compressed the gelatin sponge gently, the blood in gelatin sponge flowed and separated the mucosa of maxillary sinus, lifting height was about 3 ~ 4mm, and implanted without bone graft implantation. After 6 months, the permanent restorations were completed. After 1 years, the bone stability around the implant and the osseointegration around the maxillary sinus floor were observed. The data were analyzed by PASW statistic 18 software. Results： 36 implants were all completed permanent restorations and loaded for 1 years. The osseointegration around implants was stable. Conclusion： It is a feasible bone augmentation technique about improved maxillary sinus augmentation without bone grafting.
