上海浦东新区良性前列腺增生合并膀胱过度活动症患者生活质量调查 被引量:11

Health-related life quality in benign prostatic hyperplasia patients with coexisting overactive bladder in Shanghai Pudong New Area

摘要 目的调查上海浦东新区良性前列腺增生(BPH)合并膀胱过度活动症(OAB)患者的生活质量情况。方法运用描述性流行病学多阶段随机抽样方法,对浦东新区1 476名≥50岁BPH人群进行调查,共发现BPH合并OAB患者584名,调查BPH合并OAB患者的生活质量情况。应用国际前列腺症状评分(IPSS)、生活质量评分(QoL)、膀胱过度活动症症状评分(OABSS)评估BPH合并OAB患者下尿路症状(LUTS)情况,应用国际勃起功能指数问卷-5(IIEF-5)评估患者阴茎勃起功能情况,应用King健康问卷评分(KHQ)评估BPH合并OAB对患者生活质量的影响情况。结果年龄、前列腺体积是BPH合并OAB的危险因素。随着年龄和LUTS严重程度的增加,患者IPSS、QoL、OABSS、KHQ评分增加,IIEF-5评分下降,生活质量下降。KHQ评分较高的项目有一般健康状况(26.9±7.4)分、排尿问题严重程度(29.6±8.2)分、睡眠/精力(32.5±8.3)分。结论 BPH合并OAB严重影响患者生活质量,其影响程度与年龄、前列腺体积及LUTS严重程度呈正相关。 Objective Objective To investigate situation of health-related life quality in benign prostatic hyperplasia(BPH) patients with coexisting overactive bladder(OAB) in Shanghai Pudong New Area. Methods Methods Using multi-stage sampling of descriptive epidemiological method, 1476 men over 50 years old of BPH were investigated among general populations in Shanghai Pudong New Area. Among them, 584 BPH with coexisting OAB patients were found and their quality of life were evaluated. Lower urinary tracts symptoms(LUTS) were assessed by the Intemational Prostate Symptom Score(IPSS),quality of life(QoL) score and the Overactive Bladder Symptom Score(OABSS), and the erectile function by the International Index of Erectile Function-5(IIEF-5) questionnaire. The impact of BPH with coexisting OAB on health related quality of life was used by King's health questionnaire(KHQ). Results Results Age and prostate volume were risk factors for BPH with coexisting OAB. With the increased of age and severity of LUTS, the IPSS, QoL, OABSS and KHQ score increased, while the IIEF-5 score and the quality of life decreased. The KHQ scores showed relatively high in aspects of general health perception, the severity of urinary problems, sleep and energy. Conclusion Conclusion The symptoms of BPH with coexisting OAB have a significant effect on the quality of life. Its impact has a positive correlation with age,prostate volume and the severity of LUTS.
