第五跖骨基底骨折空心钉固定疗效观察

Observation of Curative Effect of Cannulated Screw Fixation for Fifth Metatarsal Fractures

目的探究第五跖骨基底骨折空心钉固定治疗的应用效果评价。方法选取2015年3月—2017年3月本院进行第五跖骨基底骨折治疗的224例患者,随机均分为对照组和观察组,每组112人,所选患者均符合纳入标准。观察组采用空心钉固定的方法对第五跖骨基底骨折的患者进行治疗,对照组则采取保守治疗的方法。针对两组患者骨折愈合情况,骨折愈合时间,是否存在腓肠肌刺激症状(AOFAS功能表评分、VAS评分)等方面进行对比分析。结果采用空心钉固定治疗的患者在各项功能指标评分方面优于采取保守治疗的患者。对照组的骨折愈合时间为(12.3±1.8)周,AOFAS中前足功能评分:优:57.14%、良:28.57%、中:11.60%、差:2.67%和VAS评分(1.4±0.4)分、观察组的骨折愈合时间为(10.1±2.1)周,AOFAS中前足功能评分:优:82.14%、良:9.82%、中:7.14%、差:0.89%、VAS评分情况(0.7±0.2)分,差异有统计学意义(P<0.05)。结论在第五跖骨基底骨折患者的诊疗中,加入空心钉固定的方法,可帮助患者进一步恢复,提高治疗效率,缩短治疗时间。

Objective To evaluate the effect of cannulated screw fixation for the treatment of fifth metatarsal fractures. Methods From March 2015 to March 2017, 224 patients with fifth metatarsal base fractures were treated in our hospital, they were randomly divided into the control group and the observation group, each group 112 people, selected patients met the inclusion criteria. Observation group were treated with cannulated screws fixation for the treatment of fifth metatarsal base fractures, control group was treated conservatively. The fracture healing, fracture healing time, the presence of gastrocnemius irritation symptoms （AOFAS function score, VAS score） were compared and analyzed between the two groups. Results Patients treated with cannulated nailing were superior to patients who received conservative treatment in all functional indicators, fracture healing time in the control group was （12.3±1.8） weeks, AOFAS in front foot function score： excellent： 57.14%, good： 28.57%, medium： 11.60%, difference： 2.67% and VAS score （1.4±0.4） points, The fracture healing time of the observation group was （10.1±2. 1） weeks, AOFAS in front loot function score： excellent： 82.14%, good： 9.82%, medium： 7.14%, difference： 0.g9%, VAS score （0.7±0.2） points, difference was statistically significant （P 〈 0.05）. Conclusion In the diagnosis and treatment of the fifth metatarsal base fracture, the cannulated screw fixation can help the patient recover fijrther, improve the treatment efficiency and shorten the treatment time.
