奥氮平治疗帕金森病并发精神障碍的临床疗效及安全性

Clinical effect and the safety of olanzapine in the treatment of Parkinson disease complicated with mental disorder

目的探讨奥氮平治疗帕金森病并发精神障碍的临床疗效及安全性。方法选取徐州市东方人民医院2014年5月—2016年5月收治的帕金森病并发精神障碍患者80例,随机分为对照组和观察组,各40例。对照组患者采用常规药物治疗,观察组患者在对照组基础上给予奥氮平片治疗,两组患者均持续治疗2个月。比较两组患者治疗前后精神状态量表(SAPS)评分、运动功能评分量表(UPDRS)评分、临床疗效,观察患者不良反应发生情况。结果治疗前,两组患者怪异行为、妄想、幻觉评分比较,差异无统计学意义(P>0.05);治疗后,观察组患者怪异行为、妄想、幻觉评分低于对照组(P<0.05)。两组患者治疗前后UPDRS评分比较,差异无统计学意义(P>0.05)。观察组患者临床疗效优于对照组(P<0.05)。两组患者均未出现严重不良反应。结论采用奥氮平治疗帕金森病并发精神障碍的临床疗效确切,可有效缓解患者的精神症状,且安全性高。

Objective To investigate the clinical efficacy and the safety of olanzapine in the treatment of Parkinson disease complicated with mental disorders. Methods A total of 80 cases of patients with Parkinson disease complicated with mental disorders were selected from May 2014 to May 2016 in Xuzhou Dongfang People＇s Hospital,which were randomly divided into control group and observation group,40 cases in each group. Control group was treated with conventional treatment,observation group was treated with olanzapine based on control group,both groups were treated for 2 months. The SAPS scores,UPDRS scores before and after treatment,clinical effect were compared between the two groups,and the incidence of adverse reactions were observed. Results Before treatment,no significant differences of SAPS scores of bizarre behavior,delusion or hallucination was found between the two groups（ P〉 0. 05）; after treatment,the SAPS scores of bizarre behavior,delusion and hallucination of observation group were lower than control group（ P〈 0. 05）. Before and after treatment,no significant differences of UPDRS scores was found between the two groups（ P 〉0. 05）. The observation group of clinical effect was better than control group（ P〈 0. 05）. No serious adverse reactions was happened between the two groups. Conclusion Olanzapine has an exactly clinical effect in treating Parkinson disease complicated with mental disorder,it can significantly alleviate the psychiatric symptoms of patients,and with high safety.
