嗜酸性肉芽肿性血管炎一例

摘要 患者,女,33岁,因＂反复干咳、夜间喘鸣1年余,加重伴咳痰、活动后气促1月余＂于2016年6月21日入院.患者2015年1月受凉后出现干咳、鼻塞、流涕,无发热、咳痰、胸闷、盗汗等,自服阿莫西林后好转,但仍轻微干咳,2015年6月干咳加重,并出现夜间喘鸣,无发热、咳痰、胸痛等,门诊查血常规：嗜酸性粒细胞6.0％,嗜酸性粒细胞计数0.44×109/L;呼出气一氧化氮116.8 ppb;总免疫球蛋白E（Ig E）37.71 IU/mL;肺功能：第1秒用力呼气容积（FEV1）2.3L,FEV1占预计值百分比84.3％,支气管激发试验阳性,使FEV1下降20％所需吸入乙酰甲胆碱累积剂量（PD20）0.0 119 mg;胸部X线片未见异常;鼻窦CT示：全组副鼻窦炎,双下鼻甲肥大,鼻中隔偏曲.
