养阴清肺汤联合西药治疗老年慢性阻塞性肺气肿随机平行对照研究

A Randomized Parallel Controlled Study of Yangyin Qingfei Decoction Combined with Western Medicine in the Treatment of Elderly Patients with Chronic Obstructive Pulmonary Emphysema

[目的]观察养阴清肺汤联合西药治疗老年慢性阻塞性肺气肿疗效。[方法]使用随机平行对照方法,将100例住院患者按就诊顺序号简单随机分两组。对照组50例吸氧,给予肺支气管扩张剂,糖皮质激素及抗炎、对症治疗等。治疗组50例养阴清肺汤(生地黄15g,玄参(或西洋参)13g,炒白芍15g,甘草6g,牡丹皮8g,薄荷5g,川贝母13g等;热重加胆南星5g;寒重加干姜10g;有咯血现象加白及10g,血茜草根6g),1剂/d,水煎120m L,1次/d;西药治疗同对照组。连续治疗1个月为1疗程。观测临床症状、FVC、FEV_1、FEV_1/FEV、不良反应。连续治疗3疗程,判定疗效。[结果]治疗组显效28例,有效18例,无效4例,总有效率92.00%;对照组显效15例,有效20例,无效15例,总有效率70.00%;治疗组疗效优于对照组(P<0.01)。FVC、FEV_1、FEV_1/FEV治疗组优于对照组(P<0.01)。[结论]养阴清肺汤联合西药治疗老年慢性阻塞性肺气肿,疗效满意,无严重不良反应,值得推广。

[Objective] To observe the therapeutic effect of Yangyin Qingfei Decoction Combined with western medicine on senile chronic obstructive pulmonary emphysema. [Method] 100 cases of patients were randomly divided into two groups according to the order of treatment. In the control group, 50 cases were treated with oxygen inhalation, and the patients were treated with bronchodilator, glueocorticoid, anti-inflammatory and symptomatic treatment. The treatment group of 50 cases of Yangyinqingfei Decoction （Radix Rehmanniae 15g, Radix Scrophulariae （or American ginseng） 13g, Radix Paeoniae Alba 15g, licorice 6g, peony 8g, mint 5g, Chuanbeimu 13g; TG plus bile Arisaema 5g; cold and ginger 10g; a phenomenon with Rhizoma Bletillae 10g hemoptysis, blood madder 6g） 1, agent/d, Decoction 120mL, 1 times/d; westernmedicinewiththecontrolgr oup. Continuous treatment for 1 months for a course of 1. Clinical symptoms, FVC, FEVl, FEV1/FEV, adverse reactions were observed. Continuous treatment of 3 courses to determine efficacy. [Result] In the treatment group, 28 cases were markedly effective, effective in 18 cases, ineffective in all cases, the total effective rate was 92%, the control group was markedly effective in 15 cases, effective in 20 cases, ineffective in 15 cases, the total effective rate was 70%; the treatment group was better than the control group （P〈0.01）. FVC, FEV1, FEV1/FEV treatment group was better than the control group （P〈0.01）. [Conclusion] The curative effect of Yangyin Qingfei Decoction Combined with western medicine in the treatment of senile patients with chronic obstructive pulmonary emphysema is satisfactory, no serious adverse reactions, worthy of promotion.
