乌鲁木齐市医养结合养老服务模式现状及对策研究

Study on the current situation and countermeasures of medicine-pension combined service mode in Urumqi

目的明确乌鲁木齐市"医养结合"养老服务模式现状、困境及需求,以期为推进乌鲁木齐市医养结合的发展做出贡献。方法通过问卷、访谈、实地考察、专家咨询等方法对乌鲁木齐市卫计委医养结合工作人员4名、医养结合专家库成员10名、乌鲁木齐市现有8家医养结合机构负责人以及机构工作人员25名进行医养结合现况调研。结果乌鲁木齐市年龄≥60岁人口数共计36.71万人,占户籍人口总数的16.18%,其中,失能老人4 134人,占常住老人的1.18%。各级各类医疗卫生机构1 784个,占新疆医疗卫生机构总数的11.38%。各类养老机构108个,养老床位6 179张,其中,暂无公立的老年护理院和老年病医院。执业医师和注册护士数分别为1.34万名和1.53万名。2016年乌鲁木齐市家庭医生签约达213.04万,签约率达65.77%。每千名老人机构养老服务床位17张。结论该地区医养结合养老服务的发展存在供需不平衡、医养结合总体政策体系尚未形成,缺乏政策联动体系、配套资金以及医护专业人才缺乏等现状。将医养结合养老服务延伸至社区及家庭、促进医养结合各项政策完善落实、加大医护人才培养力度等是推进该市医养结合养老服务发展进程的合理对策。

Objective To understand the current situation,predicament and requirement of the ＂medical care and combination＂pension service model in Urumqi,in order to contribute to the development of the combination of medical care in Urumqi.Methods Through questionnaire,interview,field investigation and expert consultation method,investigation regarding the current situation of the combination of medicine and pension was performed on the four staffs serving in the combination of medicine and pension in Health and Family Planning Commission of Urumqi,ten experts randomly selected from expert library of the combination of medicine and pension,heads of eight organizations in Urumqi and the relevant 25 working staffs.Results Of investigation showed that there were 0.367 million people above 60 years,accounting for 16.18% of the total registered population in Urumqi;of which,the disabled elders were 4 134 thousand,accounting for 1.18%.There were 1 784 thousand medical institutions at various levels,accounting for 11.38% of the registered medical organizations in Xinjiang.There were all kinds of 108 nursing institutions for the elder,with the number of sickbed being 6,179 thousands.of which,currently there were no public nursing care institutions or hospitals for the elders.the number of registered doctors and nurses were 13 400 and 15 300,respectively.In 2016 in Urumqi,there were 213.04 million family doctors signed,signing rate was up to 65.77%.Each thousand elderly institutions had the 17 sickbeds.Conclusion There were imbalance between supply and demand,medical and nursing combination,the overall policy system has not yet formed,which lacked of policy linkage system,matching funds and medical professionals.The combination of medical care and pension services should be extended to the community and family,thereby promoting the medical support combined with policies to improve the implementation of health care,increasing efforts to train personnel are reasonable measures to promote the city medical raising combined with the pension service development process.
