摘要 目的研究单纯收缩期高血压患者脉压与中性粒细胞、淋巴细胞的关系。方法纳入体检者中符合单纯收缩期高血压患者415例,记录一般情况、病史,测量身高、体重、体质指数、血压等,检测血常规、生化等指标。根据脉压水平分为3组:1组(50 mm Hg≤脉压<70 mm Hg)122例;2组(70mm Hg≤脉压<90 mm Hg)136例;3组(90 mm Hg≤脉压<110 mm Hg)40例。比较3组中性粒细胞计数(NEU)及百分比(NEU%)、淋巴细胞计数及百分比和中性粒细胞/淋巴细胞(NLR)水平。结果各组淋巴细胞计数及百分比比较,1组、2组、3组呈逐渐降低趋势;各组中性粒细胞计数及百分比比较,1组、2组、3组呈逐渐升高趋势;各组NLR比较,1组、2组、3组呈逐渐升高趋势,以上指标2组、3组与1组相比,差异均有统计学意义(P<0.05)。相关性分析显示,脉压与淋巴细胞计数(r=-0.137,P=0.018)、淋巴细胞百分比(r=-0.206,P<0.001)呈显著负相关;与中性粒细胞计数(r=0.135,P=0.02)、中性粒细胞百分比(r=0.335,P<0.001)、NLR(r=0.189,P=0.001)呈显著正相关。结论中性粒细胞增加和淋巴细胞减少与脉压增加相关,脉压的增加与不良心血管事件相关,NLR可较好反映机体炎症水平,其高水平可能与心血管风险增加有关。 Objective To study the relationship between pulse pressure and neutrophils and lymphocytes in patients with simple systolic hypertension. Methods 415 patients with simple systolic hypertension were enrolled. The general condition, history, height, weight, BMI, blood pressure and so on were measured. According to the pulse pressure level, patients were divided into three groups: group one (50 mmHg≤pulse pressure 〈70 mmHg, n=122); group two (70 mmHg≤pulse pressure 〈90 mmHg, n=136); group three (90 mmHg≤pulse pressure 〈110 mmHg, n=40). The levels of neutrophils (NEU) and percentage (NEU%), lymphocyte count (LYM) and percentage (LYM%) and neutrophil / lymphocyte (NLR) were compared. Results The lymphocyte count and percentage showed a decreasing trend in the order of group 1, group 2 and group 3. The neutrophil count and percentage showed a gradual increase trend in the order of group 1, group 2 and group 3. The NLR showed a increase trend in the order of group 1, group 2 and group 3. With respect of indicators above, there were significant differences between group 2 or group3 and group 1 (P〈0.05). Result of correlation analysis showed a significant negative correlation between pulse pressure and lymphocyte count (r=-0.137, P=0.018), percentage of lymphocytes (r=-0.206, P〈0.001); and neutrophil count (r=0.135, P=0.02), neutrophil percentage (r=0.335, P〈0.001), and NLR (r=0.189, P=0.001). Conclusion Increased neutrophil and lymphocyte counts were associated with increased pulse pressure. Increased pulse pressure was associated with adverse cardiovascular events. NLR could better reflect the level of inflammation and high levels of NLR may be associated with increased cardiovascular risk.
