经皮椎间孔镜椎板间入路髓核摘除术治疗L_5/S_1腰椎间盘突出症 被引量:16

摘要 目的:探讨经皮椎间孔镜椎板间入路髓核摘除术治疗L5/S1腰椎间盘突出症的临床效果。方法:对2014年2月—2017年2月采取经皮椎间孔镜椎板间入路髓核摘除术治疗的27例L5/S1腰椎间盘突出症患者进行回顾性分析,记录手术相关并发症的发生情况,定期随访进行MRI或CT复查,和术前影像对比观察硬膜囊、神经根受压情况,对患者行疼痛视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI评分)进行评估,并根据改良Macnab标准综合评价手术治疗效果。结果:27例患者均顺利完成手术,术中无并发症发生。所有患者术后均获6~12个月(平均8.5个月)随访,出院时患者VAS评分为(3.1±0.7)分,与术前(7.0±1.6)分相比,差异有统计学意义(P<0.01),末次随访时患者VAS评分为(1.8±0.6)分,相比出院时又有进一步改善(P<0.05);末次随访时患者ODI评分为(17.2±5.1)分,较术前(65.3±7.9)分有明显改善(P<0.01);末次随访时患者依据Macnab标准评定:优22例,良2例,可3例,优良率88.9%。结论:经皮椎间孔镜椎板间入路髓核摘除术治疗L5/S1腰椎间盘突出症安全有效,具有手术创伤小、出血量少、术后恢复快、不影响脊柱稳定性等优点,临床疗效较好,值得推广应用。
