摘要
目的观察定向针刺治疗对发作间期癫痫患者脑电功率的影响并探讨其机制。方法将40例间歇期癫痫患者按随机数字表法分为定向针刺组和体针对照组,定向针刺组予脑电图引导头皮针配合体针治疗,体针对照组行普通体针治疗,评估临床疗效,并通过脑电图及脑电地形图检测观察治疗前后α、β、θ、δ频段功率值的变化情况。结果两组患者发作间期致痫灶及周围脑区的α1、α2、β2、θ和δ频段的绝对功率均显著降低(P<0.05),同时健侧脑区的α1、α2绝对功率显著增加,β2、θ和δ的绝对功率显著降低(P<0.05),而定向针刺组较体针对照组改善脑电功率谱的作用更显著(P<0.05)。结论针刺治疗癫痫疗效确定,脑电图引导头皮针配合体针治疗较普通体针疗效更显著,其机制可能与其改善脑电功率谱,从而使脑电图背景渐趋稳定和顺应性相关。
Objective: To observe the effect of directional acupuncture on EEG power during epileptic seizure.Methods: 40 patients with epilepsy in interictal stage were randomly divided into the targeted acupuncture treatment group and the body acupuncture control group. The treatment group received EEG-guided head acupuncture combined with body acupuncture treatment. The control group received conventional body acupuncture treatment.The clinical curative effect was evaluated. The changes of absolute band power(ABP) of α,β,θ and δ were observed with BEAM before and after treatment. Results: In epileptogenic focus and the surrounding areas of both groups,the ABP of α1,α2,β2,θ and δ decreased compared with before treatment(P 〈 0.05). In uninjured brain regions of both groups,the ABP of α1 and α2 improved,and the ABP of β2,θ and δ decreased(P 〈 0.05). But targeted acupuncture treatment group showed more significant effect in improving EEG power spectra compared with body acupuncture control group(P 〈 0.05). Conclusion: Acupuncture has a definite effect on treating epilepsy,and EEG-guided head acupuncture combined with body acupuncture is more effective than conventional body acupuncture. It is possibly related to improving EEG power spectra and stabilizing EEG background.
