泻火平肝汤治疗急性脑出血的临床观察

Clinical Observation of Xiehuo Pinggan Decoction on Acute Cerebral Hemorrhage

目的观察泻火平肝汤对急性脑出血的临床疗效与安全性。方法将患者60例按入院顺序随机分为对照组和观察组各30例,对照组给予西医常规综合治疗,观察组给予西医常规治疗基础上加服泻心平肝汤。观察治疗前后美国国立卫生院组中量表(NIHSS)评分、日常生活能力评定量表(Barthel指数)、血肿体积以及中医证候积分变化,并评价临床疗效及安全性。结果治疗后,两组NIHSS评分、血肿体积、中医证候积分较治疗前显著降低(P<0.05),且观察组下降更明显(P<0.05);两组治疗后Barthel指数均较治疗前提高(P<0.05),与对照组相比,观察组患者日常生活能力提高更明显(P<0.05);观察组临床疗效及中医证候总有效率明显优于对照组(P<0.05)。泻火平肝汤治疗过程中,患者无严重不良反应。结论泻火平肝汤结合西医常规方法治疗脑出血,在影像学客观指标、疗效、症状改善及神经功能缺损和日常生活活动能力评分方面均具有较高的临床效价。

Objective： To evaluate the clinical efficacy and safety of Xiehuo Pinggan Decoction. Methods： 60 patients with intracerebral hemorrhage（ICH）,according to admission order,were randomly divided into the control group and the observation group,30 cases in each. The control group was given conventional comprehensive treatment of Western medicine;the observation group was added Xiehuo Pinggan Decoction. Nerve function defect scores（NIHSS）,daily life ability rating scale（Barthel index）,hematoma volume and the change of Chinese medicine symptom integral were observed before and after treatment,and the clinical efficacy and safety were evaluated. Results： After treatment,NIHSS score,hematoma volume and TCM symptom integral of both groups were significantly reduced than before treatment（P 〈 0.05）,and the observation group decreased more obviously（P 〈 0.05）. Barthel index of two groups was improved after treatment（P 〈 0.05）,and compared with control group,daily life ability of the observation group was improved more significantly（P 〈 0.05）. In clinical curative effect and TCM symptoms,the observation group was obviously better than the control group. In the course of the treatment of Xiehuo Pinggan Decoction,the patients had no severe adverse reactions. Conclusion： Xiehuo Pinggan Decoction combined with the routine treatment of Western medicine on cerebral hemorrhage has higher clinical value in imaging objective index,curative effect,symptom improvement,neurological impairment and activity score of daily life.
