RPH加外痔剥离术治疗混合痔的临床疗效

Clinical Efficacy of RPH Plus Dissection for External Hemorrhoids in the Treatment of Mixed Hemorrhoids

为探讨自动痔疮套扎术（RPH）加外痔剥离术治疗混合痔的临床疗效,将100例混合痔患者随机分成研究组和对照组,各50例,研究组行RPH加外痔剥离术治疗,对照组行传统的外剥内扎术治疗.对比2组治愈率、手术时间、术后疼痛程度、并发症发生情况,以及痔核脱落时间、创面愈合时间.结果显示,研究组术后痔核脱落时间较对照组明显延长,P〈0.001.但是2组治愈率、手术时间、术后并发症发生率比较差异均无统计学意义,P〉0.05;而且研究组术后疼痛评分明显低于对照组,创面愈合时间明显短于对照组,P〈0.001.结果表明,RPH加外痔剥离术治疗混合痔,虽术后痔核脱落时间延长,但术后疼痛轻,创面愈合时间短,而且治愈率与传统手术无差异,值得临床推广应用.

In order to explore the clinical efficacy of Ruiyun procedure for hemorrhoids（RPH） plus dissection for external hemorrhoids in the treatment of mixed hemorrhoids, 100 patients with mixed hemorrhoids were randomly divided into study group and control group,50 cases for each.The former was treated with RPH plus dissection for external hemorrhoids,while the latter received traditional method of external dissection internal ligation. The curative rate,operation time, the degree of postoperative pain, the occurrence of complications, the time of hemorrhoid defluvium and wound-surface healing time of the two groups were compared. As results,the time of hemorrhoid defluvium in the former was significantly longer than that in the latter（ P〈0.001）. However,there was no significant difference in the curative rate,operation time and the occurrence rate of postoperative complications between the two groups （ P〉0.05）. The postoperative pain score and the wound-surface healing time of the former was significantly lower and shorter than those of the latter（ P〈0.001） respectively. The results show that although using RPH plus dissection for external hemorrhoid in the treatment of mixed hemorrhoids has longer time of hemorrhoid defluvium,it has less pain after surgery and shorter wound-surface healing time,and the curative rate is also good as traditional surgery, worthy of clinical application and popularized.
