RPH辅以外痔切缝封闭术治疗混合痔的临床研究

RPH Assisted with Incision Suture Closure for External Hemorrhoids in the Treatment of Mixed Hemorrhoids：Clinical Research

为探讨自动痔疮套扎术（RPH）辅以外痔切缝封闭术治疗混合痔的临床疗效,回顾2015年3月至2016年11月我科行手术治疗的165例混合痔患者资料,其中80例行RPH辅以外痔切缝封闭术治疗（治疗组）,85例行外剥内扎术治疗（对照组）.比较2组治愈率、手术时间、术后尿潴留和Ⅱ级以上疼痛发生率、术后出血评分、肛门坠胀消退时间、创面愈合时间.结果显示,治疗组治愈率为97.5％（78/80）,对照组为95.3％（81/85）,差异无统计学意义,P〉0.05.2组患者术后肛门坠胀消退时间比较差异无统计学意义,P〉0.05.治疗组在手术时间、术后尿潴留和Ⅱ级以上疼痛发生率、术后出血评分、创面愈合时间方面明显优于对照组,差异均有统计学意义,P〈0.05.结果表明,与外剥内扎术相比,RPH辅以外痔切缝封闭术治疗混合痔操作简单,疗效确切,并发症少,创面愈合快,更加微创.

In order to explore the clinical effect of Ruiyun procedure for hemorrhoids（RPH） assisted with incision suture closure for external hemorrhoids,authors retrospectively analysed the data of 165 patients with mixed hemorrhoids who had received surgery for their hemorrhoids in authors＂ department from Mar. 2015 to Nov.2016,including 80 cases received above combination srugery as treatment group;85 cases re- ceived external dissection ~ internal ligation as control group,compared both group＇s following indexes as curative rate,operative time, postoperative uroschesis,occurence frequency of over Ⅱgrade pain, the score rating on postoperative bleeding, as well as the time for anal tenesmus disappearing and wound-surface healing.As results,in curative rate there was no statistical difference between both groups （78/80,97.5% vs 81/85,95.3%, P 〉0.05） and in postoperative tenesmus disappearing time,there was also no statistical difference（ P 〉0.05）;but in the rest indexes,the treatment was obviously superior to the control group there was statistical difference between both groups（ P 〈0.05,all）.Results show that compared with external dissection ga internal ligation, RPH assisted with incision suture closure for external hemorrhoids has such merits as simple manipulation, certain effect, less complication, faster wound-surface healing, thus being more minimally invative.
