摘要
根本原因分析及行动(RCA^2)是对根本原因分析(RCA)的进一步完善,添加了行动和过程结果管制部分,明确指出活动的最终目标是改善行动和改进效果,提高活动效率及成果有效性,避免不良事件的再次发生。本文对RCA^2的基本理念和实施步骤进行了阐述,并结合临床实际案例的运用,为广大护理管理者提供借鉴。
RCA2 is further improved basing on RCA. Actions and control parts of resultand process have been added to RCA2, which has clearly pointed out that the final goal of actions would be improvement of actions and results so as to increase action efficiency and result effectiveness as well as recurrence of adverse events. This paper described basic concepts and procedures of RCA2 combining with application of clinical practice, so as to provide reference to nursing managers.
