Research progress of discharge planning in heart failure patients

本文对出院计划的定义、内容、实施者以及出院计划在心力衰竭患者中应用的国内外现状进行综述,并针对出院计划在我国心衰患者中的实施提出建议,从而推动出院计划模式在我国心力衰竭患者中的发展。

This paper summarized the current status of discharge planning at home and abroad, including concept, contents, executors as well as the application in heart failure patients. We proposed suggestions concerning the domestic application of discharge planning in heart failure patients to promote the development of discharge planning in heart failure patients in China.
