临床路径在疝修补护理中的应用体会

Application of clinical pathway in nursing care of hernia repair

目的探讨临床护理路径在疝修补护理中的效果。方法选取2013年4月至2015年4月,湖北省恩施土家族苗族自治州中心医院行疝修补术腹股沟疝患者120例,按照随机数字表法将患者随机分为对照组59例和观察组61例,对照组予以常规的普外科护理,观察组在对照组基础上予以临床路径护理干预,比较2组患者的住院费用、住院时间、术后并发症发生情况、患者满意度等。结果观察组患者的住院费用[(3 268±356)元vs(3 924±432元)]明显低于对照组,住院费时间[(3.3±0.5)d vs(5.1±0.9 d)]明显短于对照组,差异均具有统计学意义(P均<0.05)。2组患者的尿潴留发生率、非甲级愈合发生率比较,差异均无统计学意义(3.39%vs 1.64%、5.08%vs 0.00%,P>0.05)。观察组患者的总满意度为91.80%,高于对照组的71.19%,2组比较差异有统计学意义(P<0.05)。结论在疝修补术中实施临床路径护理干预,能有效降低患者的住院费用、缩短患者的住院时间、降低术后并发症发生、提高患者的满意度。

Objective To explore the effect of clinical nursing pathway in nursing care of hernia repair. Methods From April 2013 to April 2015,120 cases of inguinal hernia underwent hernia repair in Enshi Central hospital were enrolled. The patients were randomly divided into the control group（ n = 59） and the observation group（ n = 61）. The control group was given routine nursing of general surgery,while the observation group was applied with clinical path nursing intervention. The hospitalization expenses,hospitalization time,postoperative complications and patient satisfaction,etc.,were compared between the two groups. Results the hospitalization expenses of the observation group was（ 3 238 ± 356） yuan,significantly lower than that of the control group（ 3 924 ± 432） yuan; The observation group has shorter hospitalization time than the control group（ 3. 3 ± 0. 5） days vs（ 5. 1 ± 0. 9 days）; There were no significant differences in the incidence of urinary retention（ 3. 39% vs 1. 64%） and non-primary wound healing（ 5. 08% vs 0. 00%） between the two groups（P〈0. 05）. The total satisfaction of the observation group was 91. 80%,higher than that of the control group,the difference was statistically significant（P〈0. 05）.Conclusion The clinical pathway nursing intervention can effectively reduce the cost of hospitalization,shorten the length of hospital stay,reduce postoperative complications and improve patient satisfaction.
