PFNA、DHS与股骨近端解剖锁定板治疗老年股骨粗隆间骨折的临床疗效对比 被引量:8

摘要 目的研究股骨近端防旋髓内钉(PFNA)、动力髋螺钉(DHS)和股骨近端解剖锁定板(LCP)治疗老年股骨粗隆间骨折的临床效果。方法选取我院2013年4月至2016年4月收治的90例老年股骨粗隆间骨折患者,随机平均分为PFNA组、DHS组和LCP组。观察三组患者的手术指标、治疗效果以及并发症情况。结果 PFNA组的手术时间、出血量和骨折愈合时间均优于DHS组和LCP组(P<0.05)。LCP组的手术时间、出血量和骨折愈合时间均优于DHS组(P<0.05)。PFNA组的优良率高于DHS组和LCP组(P<0.05);而DHS组和LCP组的优良率比较差异无统计学意义(P>0.05)。PFNA组的并发症发生率为3.33%,低于DHS组和LCP组(P<0.05);LCP组的并发症发生率为20.00%,低于DHS组的46.67%(P<0.05)。结论 PFNA治疗老年股骨粗隆间骨折患者的效果较好,术后恢复快,并发症少,值得临床应用与推广。 Objective To study the clinical effect of proximal femoral nail antirotation (PFNA), dynamic hip screw (DHS) and proximal femoral anatomical locking compression plate (LCP) in the treatment of elderly femoral intertrochanteric fractures. Methods 90 cases of elderly patients with femoral intertrochanteric fractures admitted to our hospital fxom April 2013 to April 2016 were selected and randomly divided into PFNA group, DHS group and LCP group equally. The operation indicators, treatment effect and complications of the three groups were observed. Results The operation time, blood loss and fracture healing time of PFNA group were better than that of DHS group and LCP group (P〈0.05); LCP group was superior to DHS group in the operation time, blood loss and fractttre healing time (P〈0.05). The excellent and good rate of PFNA group was higher than that of DHS group and LCP group (P〈0.05). No statistical difference was found in the excellent and good rate between DHS group and LCP group (P〉0.05). The incidence of complications of PFNA group was 3.33%, lower than that of DHS group and LCP group (P〈0.05). The incidence of complications of LCP group was 20.00%, lower than 46.67% of DHS group (P 〈0.05). Conclusions PFNA has better effect in the treatment of elderly patients with intertrochanteric fractures, with quicker postoperative recovery and fewer complications, which is worthy of clinical application and promotion.
