输尿管软镜钬激光碎石术对单侧输尿管上段结石术后并发症的影响

Effect of ureteroscopic holmium laser lithotripsy on postoperative complications of unilateral upper ureteral calculi

目的观察输尿管软镜钬激光碎石术对单侧输尿管上段结石术后并发症发生率的影响。方法选取2014年4月—2016年3月嵩县人民医院收治的单侧输尿管上段结石患者126例,按照入院时间分组,各63例。对照组行经皮肾镜钬激光取石术,观察组行输尿管软镜钬激光碎石术。统计对比两组手术时间、术后住院时间、术后一期结石清除率及术后并发症发生率。结果观察组手术时间、术后住院时间均少于对照组,术后一期结石清除率高于对照组,差异有统计学意义(P<0.05);观察组术后并发症发生率(7.94%)低于对照组(20.63%),差异有统计学意义(P<0.05)。结论输尿管软镜钬激光碎石术治疗单侧输尿管上段结石,结石清除效果良好,可缩短手术时间、术后住院时间,减少术后并发症。

Objective To observe the effect of ureteroscopic holmium laser lithotripsy on postoperative complications of unilateral upper ureteral calculi. Methods From April 2014 to March 2016,selected 126 patients with unilateral upper ureteral calculi in our hospital were divided into control group and observation group according to the time of admission,each group 63 cases. The control group underwent percutaneous holmium laser lithotripsy,and the observation group was treated with ureteroscopic holmium laser lithotripsy. Operation time,postoperative hospital stay,postoperative stone clearance rate at a period and postoperative complications were statistically compared between the two groups.Results The operative time and postoperative hospital stay in the observation group were significantly lower than in the control group,the postoperative stone clearance rate at a period was significantly higher than that of the control group（ P〈0. 05）; The incidence of postoperative complications in the observation group 7. 94%（ 5/63） was lower than that in the control group（ 20. 63%,13/63）,the difference was statistically significant（ P〈0. 05）. Conclusion Ureteroscopic holmium laser lithotripsy in the treatment of unilateral upper ureteral calculi,stone removal effect is good,which can shorten the operation time,postoperative hospital stay and reduce postoperative complications.
