中药饮片处方差错原因分析及点评干预效果

Analysis on the causes of prescription errors of traditional Chinese medicine pieces and the effect of comments intervention

目的对中药饮片处方差错原因进行分析,并观察点评干预效果。方法选取2015年1月-2016年12月医院门诊开具的中药饮片处方2 350张,其中在2015年1-12月未采用处方点评1 170张处方,在2016年1-12月采取处方点评1 180张处方,统计分析处方点评前后的调配差错情况及差错率。结果处方差错主要为处方不完整、调配剂量不准确、漏抓、错配、煎法错误、服用方法错误,进行处方点评前的差错率为6.41%,处方点评后的差错率为1.44%,差异有统计学意义(P<0.05)。结论处方点评能有效减少处方差错,保证患者用药的合理性和安全性,对提高治疗效果和改善医患关系有着重要的意义。

Objective To analyze the causes of prescription errors of Chinese herbal medicine and observe the effect of intervention.Methods 2 350 Chinese medicine Pieces prescriptions in the clinics of our hospital were retrospectively chosen.Among them,1 170 prescriptions were without prescription comments from January 2015 to December 2015,and 1 180 prescriptions were with prescription comments from January 2016 to December 2016.Dispatching errors and error rates of prescription reviews before and after prescription comments were taken for statistical analysis.Results Prescription errors mainly for the prescription were not complete,the deployment of the dose was not accurate,missed grasping,mismatch,fried wrong,taking wrong method.The error rate before the prescription was 6.41%,and the error rate was 1.44% after the prescription,the difference was statistically significant（P0.05）.Conclusion Prescription comments can effectively reduce the prescription error,ensure the rationality and safety of drug use,improve the treatment effect and improve the relationship between doctors and patients,and it is of great significance.
