CT引导经皮肺穿刺在周围型肺癌中的临床诊断价值分析

Clinical value of CT guided percutaneous lung biopsy for the diagnosis of peripheral lung cancer

目的探讨CT引导下经皮肺穿刺在周围型肺癌中临床诊断的应用价值。方法回顾性分析2012年7月-2015年11月螺旋CT证实有周围型肺癌患者86例,在CT引导下经皮肺穿刺活检术。对穿刺成功率,病理学检查结果及并发症进行统计学分析。结果 86例患者穿刺成功率为100.00%,病理学检查结果符合率为100.00%,患者病理分类中以腺癌为主,腺癌62例,占72.09%,鳞癌15例,占17.44%,小细胞癌7例,占8.14%,肺转移癌2例,占2.33%。并发气胸31例,并发咯血22例,同时并发气胸和咯血15例,给予积极处理均能完全治愈。结论利用CT引导经皮肺穿刺活检术对周围型肺癌进行诊断,临床诊断确诊率较高,而所出现的并发症发生率较低。

Objective To evaluate the clinical value of CT guided percutaneous lung biopsy for the diagnosis of peripheral lung cancer.Methods Retrospectively analysed 86 cases of patients with peripheral lung cancer with spiral CT confirmed from July 2012 to November 2015 in our hospital,CT-guided needle biopsy spring automatic percutaneous lung biopsy was given.The success rate,pathological diagnosis and complications were analyzed.Results The success rate of puncture in100 patients was 100.00%.The coincidence rate of pathology was 100.00%.The pathologic classification was mainly adenocarcinoma,adenocarcinoma 62 cases（72.09%）,squamous cell carcinoma in 15 cases,accounting for 17.44%,7 cases of small cell carcinoma,accounting for 8.14%,2 cases of lung metastases,accounting for 2.33%.Complicated with pneumothorax in 31 cases,hemoptysis in 22 cases,combined with pneumothorax and hemoptysis in 15 cases,given positive treatment can be completely cured.Conclusion The clinical diagnosis CT-guided percutaneous transcutaneous biopsy for peripheral lung cancer is higher,and the incidence of complications is low.
