老年糖尿病应用社区健康管理的综合控制效果分析

目的分析老年糖尿病应用社区健康管理的综合控制效果。方法选取本社区中心治疗的200例老年糖尿病患者作为研究对象,随机分组为对照组和观察组各100例;对照组采取常规治疗护理;观察组在常规治疗护理的基础上,应用社区健康管理;对比两组患者的糖尿病知晓率、治疗率、控制率,饮食依从性;根据治疗前后的FPG、2h PG、GHb A1c改善程度,综合评价病情的控制效果。结果观察组患者的糖尿病知晓率、治疗率及控制率均显著大于对照组;两组数据具有显著性差异(P<0.05);观察组患者的饮食依从性优良率为92.00%,显著大于对照组的76.00%;两组数据具有显著性差异(P<0.05)。结论老年糖尿病应用社区健康管理的综合控制效果显著,可协同提高患者的临床疗效,加强患者对病情的认知程度,改善患者的饮食依从性,值得社区推广使用。

Objective to analyze the integrated control effect of community health management in elderly diabetes patients. Methods 200 elderly diabetics who were treated in this community center were selected as the study subjects, and the group was randomly divided into the control group and the observation group. Reszdts The rate of diabetes, treatment rate and control rate of the patients in the observation group were significantly higher than that in the control group. There was a significant difference between the two groups （P〈0.05）. The dietary compliance rate of the patients in the observation group was 92.00%, significantly greater than the control group of 76.00%. There was a significant difference between the two groups （P〈0.05）. ConcluMon Elderly diabetes application of community health management of comprehensive control effect is remarkable, which can improve the coordination of clinical curative effect, strengthen the patients cognition of disease, improve the patient＇s diet adherence, worthy of clinical use.
