神经电刺激治疗卒中后吞咽障碍59例临床分析

Clinical analysis of 59 cases of post-stroke dysphagia treated by electrical stimulation

目的探讨Vitalstim神经电刺激治疗对脑卒中后吞咽障碍早期康复疗效。方法选择我院129例急性脑卒中患者,根据治疗方法不同分为研究组和对照组,对照组患者予卒中相关药物治疗及康复医师指导下吞咽训练治疗,研究组患者同时给予Vitalstim神经电刺激治疗,比较两组治疗前后吞咽功能评分及肺部感染发生率。结果研究组和对照组干预前后吞咽功能评分降低数值分别为(2.22±1.09)分和(1.30±0.89)分,有显著性差异(P=0.000)。研究组和对照组吞咽治疗总有效率分别为79.66%、57.14%,差异有统计学意义(P<0.05)。两组患者肺部感染发生率分别为28.81%、40.00%,有显著性差异(χ~2=4.074,P=0.044)。结论 Vitalstim神经电刺激联合吞咽训练可有效改善卒中后吞咽障碍早期吞咽功能,并降低患者因吞咽障碍导致的肺部感染发生率。

Objective To explore the effect of Vitalstim electric stimulation on early rehabilitation of dysphagia after stroke. Methods 129 cases of acute stroke patients were divided into study group and control group according to the treatment methods. The patients in the control group were treated with the stroke-related drug therapy and the swallowing training under the guidance of rehabilitation physician. The patients in the study group were given vitals nerve electrical stimulation treatment at the same time, and then the swallowing function score before and after intervention and the incidence of pulmonary infection were compared between both groups. Results The difference scores of swallowing function before and after intervention in the study group and control group were average（2.22 ±1.09） and（1.30±0.89） respectively, and there was significant difference（P=0.000）. The total effective rates of the study group and the control group were 79.66% and 57.14% respectively, and the difference was statistically significant（P〈0.05）. The incidence of pulmonary infection was 28.81% and 40.00% respectively, and the difference was statistically significant（ χ^2=4.074,P =0.044）. Conclusion Vitalstim electrical stimulation combined with swallowing training can effectively improve the early swallowing function and reduce the incidence of pulmonary infection in patients with dysphagia after stroke.
