氯普鲁卡因联合布托啡诺用于急诊骨科手术麻醉的效果

Anesthetic effects of chloroprocaine combined with butorphanol in emergency orthopedic surgery

目的探讨氯普鲁卡因联合布托啡诺对急诊骨科术后麻醉的临床疗效。方法选取我院急诊骨科患者85例,随机分为常规组42例与联合组43例。常规组使用布托啡诺,联合组使用氯普鲁卡因联合布托啡诺,两组均采用硬膜外麻醉,比较两组患者体征和术后疼痛视觉模拟量表(VAS)评分等指标的变化。结果插管后30 s与麻醉诱导前比较,两组患者的收缩压、舒张压和心率均降低,差异有统计学意义(P<0.05);常规组的呼吸恢复时间、睁眼时间、拔管时间均小于联合组,差异有统计学意义(P<0.01);术后2 h,常规组与联合组的疼痛视觉模拟评分分别为(3.3±0.4)分、(1.9±0.3)分,差异有统计学意义(P<0.05),其他指标无统计学意义。结论氯普鲁卡联合布托啡诺作为麻醉剂用于急诊骨科手术麻醉,能够起到良好的术前麻醉效果。

Objective To investigate the clinical efficacy of chloroprocaine combined with butorphanol in postoperative anesthesia for emergency orthopedic surgery. Methods A total of 85 patients in the Department of Emergency Orthopedics in our hospital were selected and randomly divided into routine group（n=42） and combined group（n=43）. The routine group was given butorphanol, and the combined group was given chloroprocaine combined with butorphanol. Both groups were treated with epidural anesthesia. The changes of physical signs and postoperative pain visual analog scale（VAS） score were compared between the two groups. Results The systolic blood pressure, diastolic blood pressure and heart rate were decreased in the two groups at 30 s after intubation compared with before anesthesia induction（P〈0.05）;the time of respiratory recovery, time of opening eyes and extubation time in the routine group were lower than those in the combined group, and the differences were statistically significant（P〈0.01）; at 2 hours after the surgery, the visual analog scores in the routine group and combined group were（3.3±0.4） and（1.9±0.3） respectively, which were statistically significant（P〈0.05）. Other indices were not statistically significant. Conclusion Chloroprocaine combined with butorphanol as an anesthetic in emergency orthopedic surgery can play a favorable preoperative anesthetic effect.
