摘要 目的探讨驱动蛋白家族成员2a(Kif2a)和淋巴结转移密度(ND)对乳腺癌的临床病理学意义及预后评估。方法采用免疫组织化学法检测116例乳腺癌组织中Kif2a蛋白的表达,按ND分为ND=0组、ND≤40%组和ND>40%组,分析Kif2a蛋白的表达、ND与乳腺癌患者的临床病理因素(年龄、肿瘤部位、肿瘤大小、组织学类型、TNM分期)、内分泌表型(雌激素受体、孕激素受体)、HER-2及预后的关系。采用Kaplan-Meier生存曲线分析,评价Kif2a蛋白表达、ND与乳腺癌患者的预后。结果乳腺癌组织中Kif2a蛋白的表达、ND均与患者TNM分期及HER-2的阳性表达呈正相关(rs=0.251、0.489、0.536和0.245,P=0.007、0.000、0.000和0.008),与其他临床病理因素无关(P>0.05)。Kif2a的表达与ND呈正相关(rs=0.484,P=0.000)。生存曲线分析结果显示,Kif2a蛋白表达越强、ND越高,无瘤生存时间越短(P<0.05)。结论驱动蛋白Kif2a、ND可能是乳腺癌患者预后的独立因素,其对判断乳腺癌患者的预后具有一定价值。 Objective To investigate the clinicopathological significance and prognosis of kinesin family member 2 a(Kif2 a) expression and lymph node metastasis density(ND) in breast cancer. Methods The expression of Kif2 a protein in 116 cases of breast cancer was detected by immunohistochemistry. According to the results of ND the patients were divided into ND = 0 group, ND ≤ 40% group and ND 40% group.Relations of of Kif2 a expression and ND with the clinicopathological factors(age, tumor location, tumor size,histological type and TNM stage), endocrine phenotypes(ER, PR), HER-2 and prognosis in breast cancer patients were analysed. Kaplan-Meier survival curve was applied to evaluate the expression of Kif2 a protein,ND and prognosis of the patients with breast cancer. Results Both Kif2 a protein expression and ND were positively correlated with TNM stage and positive HER-2 expression in breast cancer(rs= 0.251, 0.489, 0.536 and 0.245; P = 0.007, 0.000, 0.000 and 0.008), but not correlated with other clinicopathological factors(P〉0.05). The expression of Kif2 a was positively correlated with ND(rs= 0.484, P = 0.000). Survival curve analysis showed that the higher the expression of Kif2 a protein, the higher the ND, the shorter the tumor-free survival time(P〈0.05). Conclusions Kif2 a and ND may be independent prognostic factors in patients with breast cancer, and may have certain value in predicting the prognosis of patients with breast cancer.
