某市肺结核住院患者的药物依从性现状及影响因素调查分析

Investigation of Drug Compliance of TB Patients and Influencing Factors in a City

目的探讨肺结核住院患者的药物依从性现状及影响因素。方法随机选取2012年1月-2017年1月我市178例肺结核住院患者作为研究对象,采用问卷调查的方式记录所有患者的性别、年龄、病程及文化程度等一般临床资料和患者住院期间药物治疗的依从性,并分析肺结核患者其药物治疗依从性的现状,通过非条件单因素及多因素分析可能影响肺结核住院患者药物依从性的相关因素。结果本文结果示大部分肺结核住院患者(62.92%)药物治疗依从性较差,其中大部分为经常忘记服药、漏服药、不遵从医嘱服药及拒绝服药;通过非条件单因素分析可得,年龄、病程、文化程度、服药方式、医护人员的随访、医疗保险、婚姻状况及药物的不良反应均可能为导致肺结核住院患者其药物依从性不佳的影响因素(P<0.05);通过非条件多因素logistic回归模型分析可得,年龄≥50岁、文化程度初中以下、自服药、医护人员无定期随访、无医疗保险及重度药物不良反应均为导致肺结核住院患者药物治疗依从性不佳的独立影响因素(P<0.05)。结论目前肺结核住院患者其药物治疗依从性较差,与多种因素有关,需采取针对性措施以提高患者治疗依从性。

bObjective To investigate the current drug compliance of TB patients and influencing factors in a city.Methods A total of 178 TB patients treated in our hospital between January 2012 and January 2017 were selected.The gender,age,course of disease,educational levels and drug compliance status were recorded. Drug compliance status of these patients was analyzed. Unconditioned single factor and multi-factor logistic regression analysis was used to analyze the factors related to drug compliance of TB patients.Results The results showed that most of the TB patients（ 62.92%） had poor drug compliance,such as forgetting to take medicine,missed medication,failure to comply with instructions on the use of medicine and refusal to take medicine. Unconditioned single factor analysis showed that age,course of disease,education levels,ways medicine was taken,medical staff＇s follow-up,medical insurance,marital status and side effects of drugs were factors that caused poor drug compliance among TB patients（ P〈0.05）. Unconditioned multi-factor logistic regression analysis showed that age,educational levels,ways medicine was taken,medical staff＇s follow-up,medical insurance and side effects of drugs could be independent influencing factors of poor drug compliance on the part of TB patients（ P〈0.05）.Conclusion Inpatients with TB have poor drug compliance,which is caused by multiple factors.Necessary measures should be taken to improve compliance of patients.
