上海市农村地区家庭医生签约服务方式探讨

Discussion on Signing Service of Family Doctors in Rural Areas of Shanghai

目的探讨上海市农村地区家庭医生签约服务的方式。方法对中心自2015年起开展建立家医管理中心,家庭医生"三站式"服务方式和"3+1+1"服务模式,提高慢性病管理人数(高血压、糖尿病)、健康档案建档数,以及"延伸处方"开展后患者明显得益进行总结,阐述家庭医生签约服务在上海农村地区的方式。结果通过中心建立的这种服务方式和模式,2017年5月底前较2015年12月底前相比高血压、糖尿病、健康档案建档数分别提高了34.23%、47.89%、16.53%;延伸处方,至2016年12月开具1 437张,涉及人数440名,金额30.56万元,由于诊查费减免与药品零差率的实行,患者自付费用平均减少10.2元。结论家医管理中心,家庭医生"三站式"服务方式和"3+1+1"服务模式为上海农村地区的家庭医生签约服务中的家庭医生提供一个工作方式,有着明显的作用。

Objective This paper tries to explore the methods of contracted service among family doctors in rural areas of Shanghai. Methods Since 2015, the family doctor management center has been established： the family doctor ＂three stop＂services and ＂3＋1＋1＂ service mode, to improve the management of the number of chronic diseases（hypertension, diabetes）,health records filing number, and extension of ＂prescription＂ carried out in patients with significant benefit were summarized, and the signing service mode of family physician in rural areas of Shanghai. Results By this service mode of the center, the hypertension and diabetes, health records filing number of the end of May 2017 were increased by 34.23%, 47.89%,16.53% than those of the end of December 2015; extension of prescription issued 1 437 copies to December 2016, involving the number of 440 people, the amount of 305 600 yuan, due to the implementation of relief and drug inspecting fee zero rate, the average pay for the patient reduces 10.2 yuan. Conclusion Family doctor management center, the family doctor＂three stop＂ services and ＂3＋1 ＋1＂ service model provide a working approach for Shanghai rural family physician of the family doctor, which plays a significant role.
